POSITION TITLE: Licensed Practical Nurse (LPN)
POSITION SUMMARY:
Works in collaboration with RNs, and Unit Manager to provide high quality service to provide individualized nursing care as directed by an RN and as ordered by a physician. Provides service in patients' homes or place of residence, being respectful of the patient and their living environment. Listening to patients' preferences and recommends any changes consistent with best practice to RN.
ESSENTIAL JOB FUNCTIONS:
- Provide skilled nursing care as outlined in the nursing care plan.
- Provide nursing services and preventive procedures within the scope of LPN practice.
- Initiate preventive and rehabilitative procedures.
- Observe signs and symptoms and reports the patients reaction to treatments and any changes in their physical or emotional condition.
- Knowledgeable in teaching, supervising and counseling patients regarding their care and disease process.
- Documentation is performed accurately and timely regarding services rendered to patients, including notes on changes in condition or behavior observed.
- Assists with treatments as ordered by a physician and approved by a Home Health RN.
- Able to identify patient/environmental risk factors that would negatively impact patient care/progress.
- Participates in weekly case conferences, sharing observations or seeking problem solving for patient care.
- Advocate for the client's rights to receive appropriate care.
- Collaborates in the formulation and implementation of a comprehensive plan of nursing care.
- Collaborates with RN when acting as preceptor to new hires and students.
MARGINAL JOB FUNCTIONS:
- Performs other duties as assigned.
QUALIFICATIONS:
REQUIRED:
Strong clinical knowledge in various chronic/geriatric disease processes, possess excellent observation and interpersonal skills, ability to communicate with various levels of personnel. Implement the nursing care plan developed by an RN, skillfully performs assessment of the health status of individual patients as well as implement the appropriate strategy of care and delivers and maintains safe and effective nursing care. Have experience with electronic documentation. Valid driver's license and private vehicle or proof of reliable transportation with good driving record; and Proof of current auto liability insurance for self or driver. Accepts accountability for own practice.
PREFERRED:
One year experience.
LIC/REG/CERT:
Oregon LPN license.
NON-REG CERT:
BLS certification required by end of month from 60 days of hire and maintained.
TYPICAL PHYSICAL/MENTAL DEMANDS:
Able to work as assigned every week, have active automobile insurance, driver's license and ability to drive safely throughout the geographic area defined by Sky Lakes Home Health, in all types of weather.
PAY RANGE: $23.00 - $34.69
