The Home Health OASIS Review RN facilitates the development and implementation of a patient-centered Plan of Care based on best practices, agency protocols, and an accurate comprehensive assessment. Responsibilities include:
* Reviews OASIS (Admission, Recert and Discharge)assessment and OASIS coding for accuracy
* Ensures the Plan of Care reflects the accuracy of the OASIS assessment and primary focus. Ensures correlations between the patient's needs as identified in the clinical documentation and the resources allocated in the plan of care.
* Reviews therapy visits for correct reassessment documentation
* Educates staff on documentation to ensure compliance. Works closely with Clinical Managers to address problem areas requiring intervention that may include problematic trends in clinical documentation discovered during reviews.
Job Requirements:
* Current & valid license to practice in Oregon
* Current & valid OR Driver's license
* Current & valid car insurance
* CPR certified
* Excellent critical thinking skills
* Minimum 1-2 years clinical experience.
* Minimum 1-2 years' recent Home Health field experience preferred
* Demonstrated strength in documentation skills, both oral and written. Demonstrated ability to motivate and educate others.
* Detail-oriented and able to work independently to meet deadlines

Pay: $39.00 - $41.00 per hour
