Leads, plans and prepares an activities program for residents; ascertains needs of resident population, shapes programs, and oversees activities for residents in memory support.
Executive Director
The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
- Know and understand the vision, purpose, values and goals of Anthem Memory Care.
- Coordinates and oversees memory support activity program;
· Participate in Pathways of Purpose philosophy, knowledge of the program, and functional engagement during workday.
- Plans, prepares, directs and participates in a variety of service programs for residents, specifically addressing the needs of the cognitively impaired;
- Interviews residents to determine recreational/activity history, needs, and interests;
- Supervises and coordinates work of volunteer staff, performing administrative duties such as instructing, assigning work, reviewing work, maintaining standards, and coordinating activities;
- Schedules, promotes, leads and instructs activities; arranges the set of programs; encourages participation; coordinates in-room programs for bedfast residents and for those not desiring to attend group programs;
- Coordinates program services with director of resident services and/or other departments and committees, and facilitates communication;
- May complete quality assurance audits regarding activity documentation;
- May attend resident care meetings;
- May document residents' progress in the meeting activity plan periodically; revises as appropriate; files in medical records;
- Facilitates and attends in-service training workshops and meetings as required;
- Conducts work tasks safely and in compliance with the community’s safety program;
- Provides effective and courteous service to all residents, guests and coworkers;
- Promotes and protects the rights of all residents;
- Performs other related work as required.
College course work in social work, gerontology, psychology, recreation therapy or a related field, and one to two years of experience, preferably in a dementia setting; or any equivalent combination of training and experience that provides the required skills, knowledge and abilities.
- State certification for activities leader required (36 hour minimum);
- Requires First Aid certification;
- Serve Safe Certification
- Criminal record clearance or criminal record exemption, as required by law
- Health evaluation to ensure ability to perform duties of the position and the absence of any communicable disease, including tuberculosis and CXR clearance.
- Valid Driver’s License with a clean driving record, and free of restrictions or limitations
- Leading, planning, and promoting programs and activities;
- Maintenance and safety procedures that must be adhered to in the performance of duties;
- The use of various types of recreation and general equipment and tools;
- Arts and crafts techniques, procedures, supplies and equipment, and ability to instruct others.
- Ability to operate a variety of automated office machines including copier and computer; ability to operate a variety of audio visual, crafts and recreational equipment and tools, such as movie projector, microphones, VCR, kiln, sewing machine, cooking equipment and television sets;
- Ability to coordinate movements of fingers and hands in crafts activities, and movements of arms, legs, feet and hands in transporting residents, playing sports, working on crafts projects, exercising in an exercise class and in driving vans to transport residents;
- Must be physically able to exert a considerable amount of force occasionally and/or a moderate amount of force frequently or constantly to lift, carry, push, pull, or otherwise move objects;
- Must be able to lift and/or carry weights of patients in positioning and transporting them.
Must have ability to add, subtract, multiply, divide, and calculate decimals and percentages.
- Ability to comprehend and correctly use a variety of informational documents that may include audits, dietary lists, birthday lists, room assignments, budget summaries, telephone messages, new resident lists, room changes, progress note list and patient care plan;
- Ability to comprehend a variety of reference books and manuals including Physician's Policy and Procedure manual, patient care manual, cookbooks, craft books, recreation books and social service care plan book;
- Ability to prepare any or all of the following: attendance records, bedside and room visit records, departmental requests, progress notes, activity histories, patient care plans, petty cash requests, question and answer forms and letters of correspondence using prescribed format and conforming to all rules of punctuation, grammar, diction and style;
- Ability to use and interpret medical terminology and language;
- Ability to communicate effectively verbally and in writing with residents and families, supervisors, coworkers, students, staff, employees in the food services and housekeeping departments and the general public; ability to promote the services for the activity department among residents and families.
Must have ability to work effectively in a community environment sometimes with exposure to outdoor elements.

Pay: $40,000.00 - $55,000.00 per year
