Licensed Vocational Nurse (LVN) Hospice Continuous Care

Registry Redefined is a new registry company owned by experienced nurses redefining the meaning of care, we are currently looking to hire multiple experienced Hospice Continuous Care LVNs – We are looking to hire Licensed Vocational Nurses who are looking for the flexibility to choose their schedule and have the trust to be able to communicate with our team.

• Continuous Care LVN's works as an independent contractor for our registry where they are sent to help cover multiple hospice companies Continuous Care shifts.
• The Continuous care LVN is responsible for providing direct patient care in the patients home, responsible for collecting data and following patients plan of care. The LVN provides treatment and works collaboratively with the hospice company that they are being sent to work for.
• Participates in coordination of Hospice services for Continuous Care, appropriately reporting the identified needs to the interdisciplinary team
• Uses and prepares equipment and supplies effectively, adhering to aseptic techniques
• Performs other duties as assigned by the registered nurse or clinical leaders

PAY RATES: $ 38.00/hourly to 40.00/hourly

Licensed Vocational Nurse (LVN) Requirements:

• LVN with valid CA state license with no restrictions
• Hospice experience preferred but not required new grads welcome
• Complies with accepted professional standards and practice
• Understanding of hospice philosophy and needs of the terminally ill
• Encompasses highest Standard of Excellence approach with documentation
• Must have excellent interpersonal skills and a passion for hospice
• Demonstrates good verbal and written communication, and organization skills
• Self-directed with ability to work with little supervision, proactive nursing approach
• Current Physical and BLS, negative TB test results
• Participation in an employment work physical, if not taken one in last 6 months
• Must be authorized to work in the U.S.
• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence
