Job Title: Licensed Vocational Nurse

About the Role:
ApolloMed is looking for a Licensed Vocational Nurse to join our fast growing Dynamic Team. The Licensed Vocational Nurse is responsible for providing direct patient care under the supervision of a nurse practitioner. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes.
What You'll Do:
The Licensed Vocational Nurse will be providing direct patient care under the supervision of a nurse practitioner. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. The LVN will work directly with QCC Medicare HP triple stars measure program. QCC has a high volume of Medicare members that needs follow up after hospital discharges for medication reconciliation, Chronic Blood pressure outreach calls, Osteoporosis programs.
Qualifications:
- Provides direct patient care as defined in State Nurse Practice Act
- Implements current nursing practice following a comprehensive assessment and the plan of care
- Provide accurate and timely documentation of patient services to reflect the plan of care
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
- Participates in coordination of post-acute care services, appropriately reporting the identified needs to the interdisciplinary group
- Participate in program specifics into the essential responsibilities. Ex: MRP, OMW, CBP etc
- Uses equipment and supplies effectively and efficiently
- Reinforced hospital discharge instructions and reinforced medication adherence to the patient, patient's family and/ or patient primary caregiver. Evaluates patient's response to treatments/medications
- Reports directly to nurse practitioner for any urgent patient medical issues or conditions
- Provides patient blood draw, bone density scan, glucometer kit teaching, BP machine use
- Provide patient home visit if needed
- Participates in personal, professional growth and development. Also participates in quality assessment performance improvement teams and activities
- Performs other duties as assigned by the supervising nurse and /or nurse practitioner
- Bilingual in Mandarin or Cantonese
You're great for this role if:
- Graduate of an accredited practical nurse or vocational nursing program
- Maintains a current CPR certification
- Minimum of one-year experience in hospital setting, home health, or community health experience
- Currently licensed as an LPN/LVN in the State
- Complies with accepted professional standards and practice
- Demonstrates good verbal and written communication, and organization skills in a fast-pace environment
- Highly motivated and able to work with multiple teams simultaneously
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order
- Understands philosophy of health care managed care concept. Must be flexible and available on the weekends if needed

Environmental Job Requirements and Working Conditions:
- Oue organization follows a hybrid work structure where the expectation is to work in both in office and at home on a weekly basis. The office is located at 568 Garvey Ave, Monterey Park, CA 91754.
- Hybrid work schedule is 2 days in office per week, and 3 days work from home per week.
- The target pay range for this role is $25.00 - $30.00 per hour. This salary range represents our national target range for this role.
