Job Summary:The Licensed Practical Nurse (LPN) provides direct care of patients. Patients may include sub acute, convalescent, chronic patients or persons with disabilities. The incumbent will serve as a member of interdisciplinary treatment or other relevant teams within RiverStone Health. Primary responsibilities include assessment of patients’ needs, providing care and instruction, and communicating observations and changes to relevant team members.
Essential Functions/Major Duties and Responsibilities:A. General Licensed Practical Nurse Responsibilities 95%
Assess patient needs, answer patient questions, provide assistance and education to patients
Provide direct patient care in compliance with RiverStone Health policies as well as any programmatic policies (e.g. school district policies)
Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration.
Administer prescribed medications or start intravenous fluids, noting times and amounts on patients' charts.
Make appointments, keep records, or perform other clerical duties.
Help patients with bathing, dressing, maintaining personal hygiene, moving in bed, or standing and walking.
Work as part of a healthcare team to assess patient needs, plan and modify care, and implement interventions.
Informs primary RN, Nursing Supervisor or immediate supervisor in a timely manner of any changes in patient conditions or needs.
Documents care provided in electronic medical records according to documentation standards
Attends meetings, conferences, workshops and training sessions and reviews publications/audio-visual materials to be come and remain current on principles, practices, and new developments in assigned work areas.
Non-Essential Functions/Other duties as assigned ≥5%
Perform other duties as assigned in support of RiverStone’s mission and goals.
Minimum Qualifications
Completion of a State accredited Licensed Practical Nursing Program
Considerable patient care and nursing experience
Any combination of experience and training which provide the equivalent scope of knowledge, skills, and abilities necessary to perform the work.
Preferred Qualifications:
Direct experience with target population specific to program
Licensed as a Practical Nurse in the state of Montana and holds unrestricted License
Valid State of Montana Driver’s License
Demonstrates knowledge and skills necessary to provide care appropriate to patients of all age groups- must demonstrate knowledge of the principles of growth and development through the life span, possess the ability to assess data reflective of the patient’s status, and interpret the appropriate information needed to identify each patient’s needs
Thorough knowledge of public health principles and practices, preventive measures, and knowledge of community resources
Ability to display non-judgmental and empathetic listening skills
Ability to communicate effectively orally or in writing using technical and/or non-technical terminology
Knowledge and understanding of protected sensitive patient health information (HIPAA) and confidentiality
Ability to work collaboratively and maintain a positive work environment
