Mountain Valley has an opening for a Licensed Practical Nurse serving our serious illness patients. The team you will work with is full of people passionate about their jobs; collaborating to care for seriously ill patients in their homes or assisted living facility.
Duties
- Collect patient data to help with the admission process and ongoing IDGs
- Communicate and coordinate patient care with patients, families, referral sources, and clinical team
- Perform patient visits and ensure in-home visit fully meets the plan of care initiated by the Registered Nurse
- Identify and assess patient needs and clinical changes, accurately reporting assessment findings in a timely manner to Supervising RN or Practitioner
- Provide accurate and timely documentation, consistent with the plan of care
- Assist in keeping a variety of medical records in accordance to office procedures

Qualifications
- Must be a Licensed Practical Nurse
- Must have at least three years of experience as a licensed nurse
- Ability to utilize computers/tablets and computer software
- Medicare home health or hospice experience is preferred
- Must have a valid driver’s license
