Job Title: Licensed Clinical Social Worker
Duties:
- Adhere to the spirit of Waterfall Community Health Center to remove barriers to care and our mission statement while performing assigned duties.

- Provide Trauma-Informed Integrated Behavioral Health counseling services in an outpatient setting, including assessments, treatment planning, individual and group counseling, referrals, coordination with social determinants of health, allied agencies, and be fully integrated into the care teams.
- Utilize evidenced-based approaches to the treatment of trauma-related disorders, personality disorders, and general mental health.
- Assess patients' behavioral and mental conditions based on a review of patient information, interviews, observation, tests, and collaboration with other integrated care team members along with outside allied agencies.
- Maintain accurate and up-to-date documentation associated with comprehensive electronic client files.
- Demonstrate ability to work independently, communicate effectively both verbally and in writing, deal tactfully and patiently with clients, coworkers, and supervisors.
- Serve as a patient advocate to coordinate required services or to resolve emergency problems in crisis situations including support and advocacy to assure that the client’s medical needs and required referrals are being conveyed and that health and social services plans are being followed up on, with both providers and clients.
- Develop and implement treatment plans with integrated measurable objectives and goals based on diagnostic assessment, clinical experience, trauma or personality disorder-related certifications, and knowledge.
- Collaborates with the primary care provider in recognizing, treating, and managing mental health and psychosocial issues of patients and acts as a contributing member to the primary care team.
- Support program goals to reduce cultural and socio-economic barriers between clients and institutions committed to working with marginalized and diverse community populations.
- Perform any other duties needed to help drive our Vision, fulfill our Mission, and abide by our Organization’s Values.
- Master’s Degree in counseling, medical social work, or related field.
- Unrestricted Oregon License and Board Certified to practice as it relates to Behavioral Health and/or Mental Health Provider (LCSW, LPC, LMFT, PsyD, or PhD), required.
- One (1) or more years of experience working in healthcare or a similar setting as a behavioral and/or mental health provider, is preferred.
- Certification in any of the following are encouraged to apply: Eye Movement Desensitization Reprocessing (EMDR), Emotional Freedom Therapy (EFT), Acceptance and Commitment Therapy (ACT), Trauma-Focused Cognitive Behavioral Therapy (CBT), Treating Complex Trauma, or Dialectical Behavioral Therapy (DBT).
- Experience working in a mental health care setting with a low-income population preferred.
- Bilingual Spanish is a plus.
- Current First Aid or BLS Certification.
- Hepatitis B.
- COVID.
- Prolonged periods of sitting and/or standing at a desk working at a computer.
- Able to lift up to 15 pounds rarely.
- Able to travel as needed.
