Minimum Qualifications:
This position requires a Bachelor’s Degree in a human service related field, with one year experience providing direct services to at-risk youth and families. Lived experience in conjunction with professional experience providing direct service to at-risk youth and families may be a substitute for Bachelor’s Degree. Must have strong verbal and written communication skills. Must pass CBI, FBI, and Central Registry background checks.
Position Description:
The Care Coordinator is responsible for overall Care Coordination and Community Supervision of pre-adjudicated youth released into the community as an alternative to detention. The Care Coordinator is responsible for ensuring that each youths’ plan of care is implemented with the appropriate service referrals for needs identified through theCJRA full assessment process. The Care Coordinator is responsible for monitoring youth compliance onCommunity Supervision (tracking) and for designing Supervision Plans that meet the safety needs of the community. The Care Coordinator ensures that the Supervision Plans are individualized by taking into account youths’ CJRA risk score, the magnitude of the pending offense(s), and level of compliance demonstrated by the youth. The Care Coordinator is responsible for setting up and conducting supervision contracts and/or activation of mobile devices (such as EHM and GPS). The Care Coordinator is responsible for maintaining regular communication with service providers to gather information around compliance and needs. The Care Coordinator is responsible for building trust and rapport with the youth/family and preserving the integrity of the family unit. TheCare Coordinator is responsible for communicating with the youth/family to ensure the services in place are culturally appropriate and helping them meet their family needs. The Care Coordinator is responsible for providing and/or coordinating 24-hour emergency, on-call crisis management to the youth/family. The Care Coordinator will be responsible for writing and reporting Status and Recommendation Reports to the court. The Care Coordinator is responsible for entering information into the Colorado Trails and CYSN databases. The Care Coordinator report to the CYDC Program Supervisor.
Specific Responsibilities:
- Manage the care of all pre-adjudicated youth released into the community from detention who are receiving
CYDC funded services and/or are court ordered to be on Community Supervision.
- Provide services during non-traditional hours, to include evenings and weekends.
- Attend pertinent staffings on all youth.
- Provide daily supervision of youth in the community.
- Implement community-based level of intervention systems (incentives and consequences).
- Provide court advocacy and education to the youth/family.
- Provide timely/written summaries of progress and compliance to the court and/or referring agencies.
- Attend court hearings when necessary and appropriate.
- Provide crisis management for emergency situations.
- Maintain regular communication with youth/family and service providers.
- Have knowledge of community resources and referral processes.
- Establish/maintain relationships with community partners.
- Utilize community partners expertise and skill sets for appropriate coordination of care.
- Make referrals to service providers that meet the needs of the youth/family
- Work cooperatively with the E-ISST process to include providing referral and information to the Pathways
Partner Coordinators and actively participate in the planning process and follow through.
- Monitors youth’s progress in the services being provided.
- Enter all data into Trails and CYSN within the 48-hour timeframe.
- Maintain appropriate paperwork and client files.
- Participate as a member of the Screening Team (see screening job description)
- Conduct self in a professional manner at all times.
- Assist in coverage for employees who may be absent from work.
