POSITION

Care Navigation Support

POSITION PURPOSE

As a vital member of the Care Navigation Support Team, your primary purpose is to drive operational excellence through efficient management of the Request for Proposal (RFP) process, meticulous data management, and proactive compliance monitoring. You will play a

pivotal role in maintaining the accuracy, consistency, and integrity of Care Navigation data, ensuring adherence to healthcare policies and regulations. The work we do makes a positive difference in people’s lives for those needing long-term care due to a disability.


Data Integrity Audits

  • Conduct regular audits of Care Navigation data to validate accuracy, consistency, and compliance with established standards, promptly addressing and rectifying discrepancies.

RFP Management

  • Oversee and streamline the Request for Proposal (RFP) process, ensuring timely and accurate responses aligned with the PASCO’s services and capabilities.

Data Entry into EHR and CRM

  • Accurately input and manage essential data related to client care, caregiver demographics, eligibility, and interactions within the Electronic Health Record (EHR) and Customer Relationship Management (CRM) systems.

Compliance Monitoring

  • Monitor and uphold compliance with healthcare policies, regulations, and internal protocols, ensuring data management, privacy, and documentation within the PASCO’s meet established standards.

Documentation and Reporting

  • Maintain meticulous records of RFP responses, data audits, and compliance activities, generating insightful reports for compliance assessment and continuous process improvement.

Process Improvement

  • Collaborate with the team to identify opportunities for process improvement in data management, entry, and compliance. Propose and implement enhancements to optimize workflow efficiency.

Flex Support for Team

  • Provide flexible support during peak workload periods, assisting Care Navigation team members with data entry, documentation, and other tasks to uphold operational efficiency.

Communication and Coordination

  • Ensure effective communication and coordination within the team, sharing best practices and fostering compliance with data management standards. Ad Hoc Projects and Tasks: · Contribute to the overall success and growth of the Care Navigation Team by assisting with ad hoc projects and tasks as needed.

Key Measures of Performance for the Role

  • RFP Timeliness and Accuracy:
    • Percentage of RFPs responded to within specified timelines and accuracy of responses.
  • Data Integrity:
    • KPI: Frequency and severity of data discrepancies identified during audits.
    • Target: 99% data accuracy with less than 2% identified discrepancies in regular audits.
  • EHR and CRM Data Entry Accuracy
    • Frequency and severity of data discrepancies identified during audits.
    • Target: 99% data accuracy with less than 2% identified discrepancies in regular audits.

Qualifications


  • Excellent verbal and written communications skills.
  • Energetic, well-spoken and the ability to connect with people on the phone.
  • Strong listening skills and the ability to ask targeted questions.
  • Ability to multi-task, prioritize, and manage time effectively.
  • Ability to work quickly, efficiently and with a high degree of accuracy.

  • Strong computer skills including Microsoft Office Suite, Outlook.
  • Strong teamwork and relationship building skills
  • Experience with CRM/Salesforce preferred.
  • Experience with a healthcare documentation system preferred.
  • Experience with Medicaid or Home Healthcare experience is a plus.

Knowledge, Skills or Abilities

  • Excellent organization and time management skills, ability to maintain one’s own

schedule and meet deadlines.

  • Proven skills of customer satisfaction.
  • A fan of speed: ability to successfully operate in a fast-paced, dynamic environment

experiencing growth and expansion.

  • Moderate or exceeding ability with Microsoft Office products, specifically Outlook.
  • Familiarity with electronic health record systems.

Physical or Other Requirements (with little or reasonable accommodation):

  • Sedentary detailed work and Interacting with prospects over the phone and documenting specific details into various data systems.
  • Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.
  • Requires talking, hearing and detailed work with fingers, such as typing.
  • Requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection).

SALARY $19 - 21 per hour

LOCATION This position is hybrid. Working on site at our Lakewood facility M/Tu and remote W/Th/F 8:30a - 4:30p
