Shift: 9:00 AM - 5:30 PM
Experience:
• 24 months of Interventional Radiology experience is required.
• 3 months of experience with Epic is required.
Requirements:
• Candidates must have a Illinois license in hand at the time of application.
• This role may require floating to additional units and locations
• Candidates must live at least 60 miles away from this facility in order to be considered a traveler when applying for this role.
• COVID vaccination required for onboarding. Declinations not accepted.
• COVID booster required to apply. Declinations not accepted.
• Flu vaccination required for onboarding. Religious, medical, and personal declinations accepted.

RTO max 5 days,1 holiday.
Staff will be expected to float assignments within the Radiology and Pain departments.
COOK IS A MANDITORY FLOAT FACILITY, all resources are required to float at the facilities discretion.
Cook has the ability to request that any clinician float outside of their “home” facility as long as the floating does not exceed 50 miles. Travelers may receive notification to float 2 hours before their shift. If your clinician refuses to float, they will be subject to cancelation. There are no home units. Your clinician could be floated to a variety of units if needed.
RTO: 5 days max with no more than 2 consecutive. No RTO the day prior or after a holiday. No RTO on weekends.

Scheduling requests must be worked out with the manager directly upon starting the assignment. Any offers for this job will require a clear “Yes” or “No” response from the nurse within 24hrs or the offer must be declined.
