JOB DUTIES

  • Serves as the "point person" to facilitate incoming and outgoing behavioral health transfer from LCMC facilities, this includes transportation set up.
  • Maintains communication with all parties including but not limited to attending/accepting MD, behavioral health nurse and referring facility.
  • Reviews case with MD, nursing, accepting facilities to coordinate best transfer fit for patient based on clinical knowledge.
  • Uses problem-solving skills to determine best course of action which will be satisfactory to the patient and referral health care team to place the patient in the safest care setting.
  • Maintains streamlined communication with the appropriate physicians in order to facilitate behavioral health transfers.
  • Thinks clinically through psychiatric placement decisions while coordinating with the care team.
  • Utilizes clinical experience as a foundation for gathering accurate clinical data to assess and coordinate safe, appropriate, and prompt bed assignment of the behavioral health patient.
  • Utilizes clinical knowledge to determine appropriate facility based on psychiatric condition.
  • Documents, tracks, and coordinates logistics for hospital transfers when appropriate, and bed planning within the electronic health record.
  • Has vast knowledge of bed planning process and transfer workflow within the EHR utilized.
  • Provides details of transfer information and coordination of care.
  • Answers the phone when ringing, uses appropriate phone etiquette and guides callers through appropriate hospital process.
  • Establishes and maintains a high degree of awareness and knowledge of behavioral health workflows including the behavioral health facility services, and programs.
  • Understands how transfers are facilitated and can coordinate transfers in and out of the LCMC Health System.
  • Understands the transfer process for processing behavioral health patients and their psychiatric diagnoses.
  • Maintains professional and technical knowledge through continuous education and accepts new assignments and performs additional duties as requested such as medical transfer overflow coordination.
  • Other responsibilities will include but will not be limited to adult transfer center coordination, medical record retrieving and uploading, faxing, and printing.

License/Certification:
Required: Licensed EMT, NREMT, EMR in the State of Louisiana.
Experience:
Minimum Required: 3 years medical field experience as an EMT, Paramedic, NREMT, or with Electronic Medical Record (EMR) experience.
Preferred: Critical care and/or emergency, behavioral Health, or patient care technician experience.
Special Skills/Training:
  • Knowledge of Behavioral Health with high level critical thinking ability.
  • Exhibits excellent customer relationship skills including telephone and personal communication with all customer groups (physicians, administrators, nurses, case managers, and other team members).
  • Proficient in hospital computer systems for accurate data collection, monitoring and evaluation
