Ihh Rn Care Manager

Integrated Health Home Nurse Manager functions collaboratively with the IHH Team and the other disciplines to provide and enhance patient care. The IHH Nurse proactively strives to maintain and improve quality outcomes. The IHH Nurse Manager is responsible for facilitating patient care, anticipating patient care needs, individualizing patient care and adjusting or modifying the nursing plan of care as appropriate.

Job Duties Synthesize information from treatment records, risk stratification, and other sources to assess program enrollees. Applies evidence-based clinical guidelines for behavioral and physical health issues Collaborate with Health Home Team and treating providers in developing and implementing care coordination plan to address treatment barriers for each high-risk member Maintain HIPAA compliance and adhere to all company policies Monitor the high-risk care coordination plan and facilitate care and communication amongst treating behavioral and physical providers Develop symptom response and crisis plans, ensuring more appropriate use of physician visits rather than emergency room and hospital admissions Promote patient's health and ensure that all personal health goals are included in person-centered care management plans Provide health education to patients on preventing and managing chronic conditions using evidence-based sources Support day to day collaboration between physical health and behavioral health provider Understand and Implement Population Management strategies *List of duties is not intended to be all-inclusive and may be expanded to include other duties and responsibilities at anytime by the supervisor.
