We are hiring a Hybrid Case Manager (LGSW or RN).
The Care Manager is responsible for providing comprehensive care coordination services to members who have moderate to complex medical and psychosocial needs enrolled in the health plan.
The candiiate just be a Licesned Social Worker(LGSW) or Registered Nurse(RN) in DC to qualify.
Requirements: Active LGSW or RN License in DC
Hours: This position is Full time, Monday through Friday Day time 8:30am-5pm. There are no part time opportunities available at this time.
Job Type: This position is Hybrid in Washington District Of Columbia 20005; a mix of field day and remote work - - flexibility given.
Location: District of Columbia (DC licensing required)
Pay: $32-50 per hour (depends on experience and licesne)
Job Details:
- Develop and implement individualized care plans for children and youth with special healthcare needs.
- Work with children and caregivers for those with special health needs (ages 0-36).
- Conduct thorough assessments of the physical, emotional, and social needs of each member.
- Provide emotional support and guidance to help them navigate complex healthcare systems, community resources, and entitlement programs.
- Generate reports to inform healthcare professionals and insurance providers about the client's progress and needs.
- Provide education and training to clients and their families on topics related to healthcare management, disease prevention, self-advocacy, and available resources.
