Position Summary:

Us Helping Us is a mission-driven nonprofit known for its work at the intersection of culturally appropriate service delivery, community education and mobilization, advocacy, and research. For the past 30 years, Us Helping Us has provided innovative services, programs, and activities to improve the health and well-being of racial, sexual, and gender minorities, especially Black sexual minority men, people with HIV (PWH), and persons with substance use and mental health disorders. As it exists today, Us Helping Us offers a variety of clinical, behavioral, and social services in a trusting, affirming, and inclusive environment. Importantly, Us Helping Us is a multi-service organization that is client-centric and provides coordinated, compassionate services that assist clients in realizing their full potential. We are currently hiring a Care Coordination Managerwho fosters client care activities, exchanges information with a diverse Care Team concerned with a client's care, strives to achieve more effective care, and is committed, customer-oriented, and independently motivated person with the necessary skills to fill this position.

Position Summary:

Reporting to the Director of Clinical Services, the Care Coordination Managerwill have the following responsibilities:

Client caseload management:

  • Conduct intake assessment, needs assessment, treatment planning, and reassessment services by assessing clients’ essential social support needs, life situations, and support networks to understand how these factors may impact their engagement in medical care.
  • Review client cases with Case Managers, Community Health Workers, and the Care Team, and provide advice, direction, and support as needed.
  • In most cases, organize or lead client case management training sessions.
  • May meet with the client along with the Non-medical Case Manager after primary care physician appointments to review and update the care plan.
  • Advocate for and refer clients to community resources (Medical Case Management, emergency food assistance, PrEP) that will improve clients' overall health and well-being.
  • Respond to crisis situations, such as intimate partner violence and abuse, and be knowledgeable of available community resources and/or evidence-based treatments.
  • Work to build the trust and confidence of clients to overcome any fears or anxiety related to their health condition(s) or with navigating the healthcare system.
  • Work closely with Community Health Workers to implement Individualized Treatment Plans for clients and to identify services to address other aspects of their lives, such as mental health and substance abuse treatment, domestic violence counseling, housing programs, and education and employment services.
  • Follow-up with clients to ensure they attend their appointments, fill their prescriptions, take their medication(s) as prescribed, and/or address any barriers or challenges to adhere to their medication(s) and remain engaged in care, among other things.
  • Initiate outreach and missed appointment procedures, as necessary, and facilitate the re-engagement of clients lost to follow-up.
  • Maintain confidential, up-to-date client contact and fulfillment of treatment goals and help clients adjust to changes and challenges in their lives, such as medical diagnosis or illness, divorce, or unemployment.

Organizational and administrative duties:

  • Provide day-to-day support, supervision, and performance reviews for Non-medical Case Managers.
  • Facilitate Care Team meetings with Non-medical Case Managers, Community Health Workers, Clinical team, and other health care providers to discuss client Care Plans and share information regarding referral sources.
  • Establish and retain client referral systems from Care Coordination systems
  • Attend and represent the organization at professional conferences, in-service training, and meetings at the request of or with the approval of the supervisor
  • With guidance from the quality and evaluation team, conduct quality assurance and monitoring activities for service delivery and documentation.
  • Perform other duties as assigned.

Preferred Knowledge, Abilities and Skills:

  • Excellent communication (written and verbal), active listening, and interpersonal skills are essential.

  • Sensitive to others' needs and feelings, especially LGBTQ youth and adults; honest and ethical; reliable, responsible, and dependable; and willing to take on responsibilities and challenges.
  • Plan and organize work in an effective and timely manner and, in a team environment, maintain a flexible schedule in accordance with program and organizational needs.

Required Experience and Education:

  • Master’s Degree (Masters in Social Work) required. LMSW/LGSW/LCSW preferred.
  • Minimum of 2 years of case management experience; HIV case management preferred.
  • Ongoing education/training in HIV/STI and chronic disease-related subjects.
  • The agency will train new hires regarding confidentiality, client rights, and grievance procedures.
  • This position requires intermediate computer skills required, including proficient use of Microsoft Office Suite and electronic health records.
  • Commitment: Passion for the mission of the organization as well as linkage and retention-in-care services, including a strong commitment to working to improve the health and quality of life of LGBT youth and adults, people living with HIV/AIDS, other chronic diseases, and mental health disorders.
  • Knowledge: Must have demonstrated knowledge of a variety of health-related services in DC, Maryland, and Virginia, along with an interest and ability to expand knowledge through training.
  • Experience: Must demonstrate experience with issues related to HIV care and treatment. Experience with issues that put people at risk for poorer health is also highly helpful, such as homelessness, incarceration, domestic violence, and/or substance abuse.
  • Skills: Strong communication skills are essential. Demonstrated ability to conduct sensitive, empathetic interviews and conversations that respect the dignity and diversity of our clients. Ability to engage staff and clients in a supportive and empowering way. Ability to help manage conflict. Ability to maintain client records and information in an accurate, timely and confidential manner.
  • Personal qualities: Personal qualities include energy, intelligence, sound judgment, the ability to work independently (with less supervision), the ability to work effectively in a busy environment, and the interpersonal skills to work effectively in partnership with others.

Specifications:

  • Salary range $65,000-$85,000 (based on experience) plus a generous benefits package, including annual leave, 80/20 employer/employee-paid health/dental/vision insurance, and federally observed holidays.
  • Work a minimum of 40 hours per week, including evenings and weekend commitments.
  • Valid U.S. work permit required.
  • Must submit to and successfully complete a background investigation and drug test.
  • Us Helping Us has a smoke-free environment, and non-smokers are preferred. Excellent personal hygiene is a must.
