Duties:
- Direct the development and execution of our clinical care program, lead the nursing and interdisciplinary team to ensure the delivery of patient-centered care
- Provide compassionate and comprehensive care to patients in their homes and maintain compliance with all regulations
- Assess patient needs and develop individualized care plans
- Administer medications and treatments as prescribed by the physician
- Monitor patient vital signs and report any changes or concerns to the healthcare team
- Assist with pain management and symptom control
- Educate patients and their families on disease processes, treatment options, and end-of-life care
- Collaborate with interdisciplinary team members to ensure holistic care for patients
- Maintain accurate and up-to-date patient records
Qualifications:
- Valid nursing license (RN) in the state of Texas
- Minimum of 1 year of nursing experience, preferably in hospice or palliative care
- Knowledge of Medicare guidelines and regulations related to hospice care
- Excellent communication and interpersonal skills
- Ability to provide emotional support to patients and their families during difficult times
- Strong critical thinking and problem-solving abilities
- Compassionate and empathetic nature

Pay: $45.00 - $90.00 per hour
