The ideal candidate has excellent clinical skills, a passion for direct patient care, and the ability to function well autonomously. Travel to San Antonio and surrounding areas is necessary. Duties & Responsibilities:
- Evaluate and treat patients using the most current technology and practices
- Develop initial plan of care based on communication with the physician and patient assessment
- Participate in clinical outcomes monitoring, follow up, and agency performance improvement initiatives
- Manage and educate Hospice Aides and LPNs performance in implementing nursing services
- Collaborate with an interdisciplinary team to provide highest level of care
