Job Description:
Bella of Texas is currently seeking a full-time Registered Nurse (RN) to join our Home Health line of service. The ideal candidate will possess a passion for providing exceptional care to patients in the comfort of their homes.
Responsibilities:
- Conduct home care visits in select senior care communities in the DFW area for various purposes including SN initial assessments, OASIS admissions, re-cert visits, Sup & SN visits.
- Ensure accurate and thorough documentation utilizing Home Health forms, OASIS charting, and adhering to Medicare guidelines, rules, and regulations to ensure compliance with all State regulations.
- Demonstrate proficiency with electronic charting systems, particularly Axxess, to maintain accurate and up-to-date patient records.
- Collaborate effectively with interdisciplinary teams to develop and implement comprehensive care plans tailored to each patient's individual needs.
- Provide patient and family education regarding healthcare management, disease prevention, and treatment options.
Requirements:
- Valid Registered Nurse (RN) license in the state of Texas.
- Proficient knowledge and experience with Home Health forms, OASIS charting, Medicare guidelines, rules, and regulations.
- Familiarity with electronic charting systems, particularly Axxess, is preferred.
- Strong interpersonal and communication skills with the ability to work effectively in a multidisciplinary team environment.
- Compassionate and patient-centered approach to care delivery.
- Ability to adapt to changing environments and prioritize tasks effectively.
- Valid driver's license and reliable transportation for travel within the DFW area.
