The Hospice Case Manager (RN) assumes the responsibility for identifying patient/family needs and for providing palliative care in accordance with the attending physician's orders and plan of care.
The qualified candidate will possess the following:
A. Education
1. Graduation from an approved school of Registered Professional Nurses
B. Experience
1. At least two years of varied work experience as a professional nurse, and preferrably one of the two years in an acute care setting
2. Demonstrated ability to access and respond to the needs of patients and families in varied settings
3. Skilled in nursing practice and able to cope with patient/family emotional stress
C. Licensure/Certification
1. Registered Nurse, currently licensed in Georgia
2. Current valid driver's license and ability to drive own car
