Responsible for educating physicians, acute care facility staff and other referral sources regarding the Home Health Care role in accordance with Agency policy. Functions as agency liaison for the referral process. Participate in continuous quality improvement activities and educational experiences in support of Agency philosophy and objectives, as well as, Health System initiatives.
EDUCATION/TRAINING:
Graduate of an accredited School of Nursing/College. BSN preferred. At least one year professional nurse experience within the last five years in an acute care setting. One year of experience in home care and hospital discharge planning preferred.
LICENSURE/CERTIFICATION:
Valid RN license. Valid State driver's license required.
EXPERIENCE/SKILLS/ABILITIES:
Good organizational skills. Knowledge of the principles of growth and development over the life span. Ability to effectively communicate orally and in writing with physicians, patients, family and staff and with resources within the community. Successful completion of annual clinical, age and job specific competencies and skill verification tools required.
