Position Summary
General Summary/Responsibilities:
The Home Care Liaison is responsible for identifying and coordinating services for Home care including durable medical equipment and infusion. The Home Care Liaison is responsible for the communication of new programs and Home care services to hospitals , physicians , staff and patients. The Home Care Liaison is accountable for assisting in the development of strategies to educate ICM staff and physicians regarding clinical programs , as well as conducting an annual review of clinical assessment skills for each RN/LPN.
Dimensions:
Ensures services are arranged for patients requiring Home care services , durable medical equipment and infusion. Responsible for executing strategies identified as priorities by the organization. Actively educates hospital and physician staff in programs and services. Regularly problem solves and communicates opportunities for service enhancement to manager.

Education
Minimum Level of Education Required: Associate degree

Current Ohio RN or LPN License.
Experience

Preferred experience: One year experience in clinical/ medical setting. Home health experience preferred.
