Erie Family Health Centers is looking for a valuable addition to our health education team.
This position is a one year on-the job training position that will be compensated at $16 an hour.
General Summary:
The Health Educator will provide support and education to patients across Erie including students, immigrants, low-income individuals, and those experiencing hardships. The Health Educator will provide students with vital information regarding their own health and how to advocate for themselves in the health care system.
The Health Educator will provide sexual health, substance use prevention, and nutrition education to students and patients, facilitate health campaigns and outreach that promote Erie services, support transitions to adult care, and coordinate external services, such as dental and vision, for students. The Health Educator will also contribute to Erie-wide efforts around education, outreach, and resource navigation.
This position is a paid year-long on-the-job training program that includes on-the-job mentorship that will build the leadership capacity in the candidate.
Responsibilities:
Home Team Responsibilities
- Coordinate 1:1 nutrition/health coaching meeting with students
- Implement health education curriculums to students in schools and community organizations
- Arrange for external health partners, like vision and dental partners to conduct school visits
- Host health promotions activities in the lunchroom and at school events and post health promotion materials in school
- Support promotion, recruitment and engagement of Erie’s School Based Advisory Board and the Youth Advisory Committee
Centralized Responsibilities
- Participate in health fairs, awareness, and community partnership or coalition meetings
- Assist in community events promoting agency service
- Provide basic health education and resource linkages
Leadership Development Responsibilities
- Attend all Nourish and Process Improvement trainings
- Participate in Community Health Mentorship Program
- Participate in Service Year Professional Development Opportunities
- Attend monthly meetings, bi-monthly lunch and learns and annual showcase
- Meet with community health mentor
Skills and Knowledge:
- Proficient computer knowledge, skills, and experience (Microsoft Office)
- Bilingual fluency (English/Spanish) a plus (may be required to assist or work with non/English or Spanish speaking only patients and/or guests)
Experience:
- Comfortable presenting in front of large groups of people or interest in learning
- Experience working with underserved populations (preferred) or willingness to learn

- At least 18 years of age by program start date
- Legally eligible to be employed in the United States
Education:
- High School diploma or equivalent required
- Full time position with benefits. Pay is $16 an hour.
