Home Health Care Professional

Job Overview:

We are seeking compassionate and dedicated Home Health professionals. The current positions needed are: PCAs, CNAs, LPNs, and RNs. Some of your roles may be, but are not limited to:

- Providing direct patient care, including administering medications, monitoring vital signs, and assisting with personal hygiene

- Develop and implement care plans based on individual needs and preferences

- Collaborate with other healthCare Professionals to ensure comprehensive and coordinated care

- Educate patients and their families on managing illnesses and promoting overall wellness

- Assist with meal preparation, ensuring dietary needs are met

- Perform light housekeeping duties to maintain a clean and safe environment

- Document patient information accurately and timely in electronic health record systems

Experience:

-Please have verifiable experience (Willing to train the right candidates).

- Experience working with individuals with developmental disabilities is a plus
