Duties:
- Conduct assessments and evaluations of patients to determine their social and emotional needs
- Develop and implement care plans based on patient assessments
- Provide counseling and support to patients and their families to address emotional, social, and psychological issues related to their medical condition
- Assist patients in accessing community resources and support services
- Collaborate with other healthcare professionals to ensure coordinated care for patients
- Advocate for patients' rights and ensure their needs are met within the healthcare system
- Provide education and guidance to patients and their families on coping strategies, self-care, and available resources
- Schedule skilled staff to see patients
Experience:
- Bachelor's or Master's degree in Social Work or related field
- Previous experience working with individuals with dementia, developmental disabilities, autism, PTSD, or in senior care settings is preferred
- Knowledge of medical terminology, healthcare systems, and community resources
- Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare professionals
- Ability to assess complex situations, develop care plans, and provide appropriate interventions
- Proficient in conducting research related to social work practice
