Essential Duties and Responsibilities:
- Communicate with family members and other agency personnel to adequately plan for and provide patient's needs.
- Assist in personal hygiene.
- Helping with general light housekeeping
- Assisting with Ambulation and exercise
- Reporting changes in client's condition
- Attend and participates in continuing education programs.
- Drive in own car to and from patient's home. Work on patients home or community setting
Requirements:
- Driver's License and Valid Auto Insurance
- Licensed by state of IL (IDPH)
- CPR Certification
Experience:
- 1-2 years' experience in Home Health Preferred
