Job Summary
This position reports to the Home Care Supervisor PACE and provides direct patient care in the community and at the PACE Center.
Accountabilities

• Provides personal care to include, bathing, dressing, feeding, toileting, transfers, lifting and incontinent care, to the PACE participants in their homes. - 60%

• Provides care to participants at the PACE Center. - 20%

• Responsible to report any changes in medical or physical status to the Home Care Supervisor PACE or designee. - 10%

• Performs procedures/treatments under supervision of HCS or RN. - 10%

Supervisory/Management Responsibilities
This is a non-management job that will report to a supervisor, manager, director or executive.
Minimum Requirements

• High School Diploma or equivalent
• 1 year - Nurse tech hospital experience or skilled nursing facility. Must have 1 year experience working with the frail or elderly.

Required Certifications, Registrations, Licenses

• SC CNA
• CPR
• Valid driver's license, an acceptable three-year motor vehicle record as defined by the Acceptable Motor Vehicle Record (MVR) Chart, and a certificate of auto insurance.

Required Knowledge, Skills, Abilities

• Basic computer skills
• Knowledge of office equipment (fax/copier)
