Home Care Hospital Liaison

Job Description:
Baptist Home Health is looking for a Hospital Liaison to join their team in Paducah, KY.
The Hospital Liaison is responsible for census development through marketing and educating potential referral sources on the benefits of Baptist Health Home Care. The Hospital Liaison serves as a liaison between the Home Health Agency and hospital case managers, physicians, patients, and families to ensure continuity of quality care from the hospital setting to the home.
This position provides a visible presence and ensures appropriate utilization of Home Care services. The Hospital liaison provides education to the case managers and other internal referral sources regarding home health care including hospice and home infusion where appropriate
Principal Duties and Responsibilities:

• Works in conjunction with the agency intake team.
• Screens referrals and meets with patient/families.
• Accepts referrals for services and enters intake information into Home Care software system according to procedure.
• Evaluates referral appropriateness this may include a review of medical records and assessments of patient documentation to determine what services are needed.
• Relays accurate and complete patient information to Home Care/hospice nurse mangers prior to discharge from a hospital to facilitate a smooth transition.
• Monitors hospital hold, pending reports to track referrals.
• Secures all information needed to process a complete referral and scans information into Homecare software.
• Arranges for delivery of needed equipment through DME vendor that the patient selects when needed.
• May participate in administrative on call rotation (agency specific)

Minimum Education, Training, and Experience:

• Current licensed nurse in the state of agency location or a Bachelor's Degree in Public Relations, Marketing, or health related field, preferred
• Two years' experience in a healthcare setting.
