Medical Case Manager

The Position: Within agency policies, under the direction of the Medical Case Manager Supervisor, the Medical Case Manager for HIV Care carries out a variety of Medical Case Management duties involving direct patient contact in clinic and during home visits. The incumbent is a professional who works in collaboration with the HIV Medical Case Management team. The incumbent usually works on a one-to-one client-worker basis to assist persons living with HIV/AIDS to independence in decision-making and accessing services for their health-related or psychosocial needs. The incumbent will also work with clients who were formally engaged in more intensive service levels and/or who have progressed to self-management. Duties include assessing the sufficiency of self-management and providing additional services when appropriate, to prevent lapses in care.
The ideal candidate is a dedicated public health professional, who has a passion for public health, enjoys working with others, and who brings humility and initiative to their work.
Hours: 40 hours per week - 100% FTE, requires occasional evening and weekend hours.
Status: Regular - Permanent full-time position.

Benefits include generous paid time off, and twelve paid holidays per year. Employees participate in the Public Employees Retirement System (PERS), a defined benefit retirement plan managed by the Washington State Department of Retirement Systems and Social Security. Several deferred compensation retirement savings plans are also offered.
Other: This position requires in-person interactions with staff and members of the public.
Please note: After a successful initial trial service period, generally at least three months, a limited hybrid remote work arrangement may be established as mutually beneficial.

Essential Functions
  • Conducts intake/assessment interviews with HIV/AIDS clients who may be self-referred or referred by another community agency.
  • Develops a plan of service for individual clients. Provides prevention and risk reduction education for negative partners, friends, and/or family members as needed.
  • Organizes and manages caseload and referrals; maintains open communication and acts as a client advocate with involved agencies and other service providers.
  • Establishes relationships with health care and social service agencies to assist/coordinate with discharge planning, referrals, and continuing care.
  • Coordinates prevention case management issues with case management team and peers. Participates in clinical staffing and administrative meetings.
  • Coordinates client participation in appropriate educational and social activities.
  • Maintains records on all client encounters including home and office visits, telephone calls, and contact with client's providers congruent with the Health District and the Office of Infectious Disease Washington State Department of Health HIV Community Services case management guidelines.
  • Uses the appropriate electronic medical documentation system proficiently and regularly for all client records as required.
  • Maintains data for required reports as requested.
  • Works closely with HIV Clinic staff and clinical staff regarding client medical care.
  • Provides post-test counseling for patients who test positive.
  • Provides risk reduction counseling as requested.
  • Represents KPHD HIV/AIDS services in public outreach opportunities as requested. Participates in associated education and outreach efforts as requested.
  • Works with the Case Management Team to maintain, refine, and comply with protocols, procedures, and referral/resource information files.
  • Completes timely and accurate random moments to comply with contractual requirements of Medicaid Administrative Match.
  • Keeps current on and complies with HIPAA and other privacy regulations, ensuring confidentiality always.
  • Accurately completes and submits electronic timecard on a weekly basis as required.
  • Responds to public health emergencies as required by the District.
  • Demonstrates cultural competency and embeds health equity into all aspects of assigned work and associated interactions.

Work Environment and Physical Demands
  • Work is performed both indoors in an office environment and in the field throughout the 4-county region (conducting home visits, etc.).
  • Requires the ability to communicate with others orally, face to face and by telephone.
  • Requires manual and finger dexterity and hand-eye-arm coordination to write and to operate computers and a variety of general office equipment. Requires mobility to accomplish other desktop work, retrieve files, and to move to various District locations. Requires visual acuity to read computer screens, printed materials, and detailed information. Essential duties may involve occasional kneeling, squatting, crouching, stooping, crawling, standing, bending, climbing (to stack, store or retrieve supplies or various office equipment).
  • May occasionally be required to work a varying schedule which may include evenings and weekends.
  • Duties require carrying a cell phone or electronic device.
  • Exposure to individuals from the public who are upset, angry, agitated and sometimes hostile, requiring the use of conflict management and coping skills.
  • Frequently required to perform work in confidence and under pressure for deadlines, and to maintain professional composure and tact, patience, and courtesy always.
  • The environment is dynamic and constantly changing, resulting in continually re-evaluating and shifting priorities.
  • The incumbent is required to use the combined senses of touch, sight, smell, and hearing during on-going client assessment. Duties require the ability to read typewritten and/or handwritten orders and notes.
  • Off-site visits require the incumbent to carry supplies and/or equipment up and down stairs as well as into and out of a vehicle daily.
  • While performing home visits, the incumbent may encounter unsanitary conditions, animals (pets) and clients and/or family members who are smoking, exposing the incumbent to second-hand smoke which may cause allergic reaction and/or illness.
  • The incumbent may be exposed to emotionally disturbing experiences. The incumbent is expected to remain calm, controlled, and professional, regardless of the situation and to demonstrate care and compassion to the client, family member and other members of the health care team.
  • While conducting home visits, the incumbent may be exposed to potentially violent clients and/or family members who are under the effects of alcohol or drugs, are mentally unstable or homeless. As a professional obligation, the incumbent is required to report abuse if it is encountered which puts additional stress on the incumbent. At times, the emotional state of family members brings uncertainty in knowing what to expect while at work. As a result, there is legitimate concern about one's own safety as well as being responsible for the lives of patients and their families, risk of assault and unknown and unpredictable situations.
  • Regularly the incumbent will be required the ability to lift/and or carry objects and materials up to ten pounds (taking infant scale into patient homes during a visit). Occasionally, the incumbent in this position may be required to lift/ carry objects up to twenty-five pounds.
  • May be required to stay at or return to work during public health incidents and/or emergencies to perform duties specific to this classification or to perform other duties as requested in an assigned response position. This may require working a non-traditional work schedule or working outside normal assigned duties during the incident and/or emergency.

Education & Experience
Standards for Personnel per Contract
  • Understand Statewide HIV Case Management Standards and all requirements.
  • Have knowledge of HIV/AIDS care service delivery system.
  • Have education, knowledge, and skills sufficient to fulfill the functions and scope of the position.
  • Receive regular, direct supervision.
Agency Classification Minimum Requirements
Social Worker 1
  • Bachelor's degree in social work, counseling, psychology, sociology, or related field and one (1) year of related experience,
Any equivalent combination of education, certification, community service, training, and life experiences that demonstrates competency may also be considered.
Degrees must be from appropriately accredited institutions.
Licenses, Certificates & Other Requirements
  • Performance of job duties requires driving on a regular basis throughout the region, a valid Washington State driver's license, the use of the incumbent's personal motor vehicle, and proof of appropriate auto insurance.

Medical, Dental and Vision Coverage
For eligible full-time employees (40 hours per week):
  • The District currently pays from 85% to 100% of the premiums for medical coverage each month, depending on which plan the employee selects and the number of persons they choose to cover.
  • The District pays 100% of the dental premiums.
  • Employees with other coverage may elect to waive medical and/or dental coverage and instead receive a monthly deposit to a Health Reimbursement Account (HRA) for out-of-pocket medical, dental, and vision expenses; because HRAs carry over from year to year, they can provide tax-free savings for retirement health care costs.

Life and Disability Insurance
We also provide other benefits, such as a $48,000 term life insurance policy and a long-term disability policy. Employees may choose to purchase additional life and disability insurance. Employees may, at their option, purchase additional life or long-term disability insurance, dependent life insurance, accidental death and dismemberment insurance and, short-term disability coverage.
Retirement and Savings Plans
Eligible employees participate in the Public Employees Retirement System (PERS) a defined benefit retirement plan managed by the Washington State Department of Retirement Systems. The plan is funded by a combination of employer and employee contributions. In addition, we offer a choice of three 457 Deferred Compensation retirement plans that allow individuals to set aside a portion of their income for retirement on a tax-deferred basis. These plans are similar to 401-K plans offered by for-profit employers.
Paid Time Off
We offer generous paid time off, with "general leave" that begins with 18 days per year for eligible full-time employees and increases to 31 days per year after 25 years of service. "General leave" is used for vacation, personal appointments, minor illnesses, etc. For longer term illnesses, injury or pregnancy, we also provide 6 days of "extended leave" per year. In addition, we have 11 paid holidays per year, including the day after Thanksgiving and a personal floating holiday. We offer bereavement leave, all state and federally mandated leaves such as family medical leave, military leave, and other leave categories that help our employees manage significant life events.
