We are looking for HIV Medical Case Manager Leadto join our clinics! Under the direction of the HIV Program Manager and HIV Program Clinical Lead, the HIV Case Manager Lead (MCML) provides a full range of medical and supportive case management services to People Living with HIV (PLWH) including those who have complex medical and social needs, have multiple diagnoses, are living with a disability, are unstably housed, or have other significant barriers to care. In collaboration with other members of the healthcare team, the MCML links patients to medical and support services by organizing, coordinating, and providing case management services to clients within the HIV Program who need help accessing medical, social, community, or any other resources. In addition, the Case Manager performs intakes, comprehensive assessments and develops service plans as specified by Title XIX Case Management Standards, identifies eligibility for medical, social, and community resources and assists patients with applications for programs such as insurance, SNAP, unemployment, medication assistance, and emergency financial assistance. Plus, other duties as assigned.
Qualifications
- Bachelor’s degree or equivalent from an accredited college or university with major course work in social work, sociology, public health, or related field.
- Demonstrated understanding of statewide HIV case management standards and all requirements.
- Three years of case management experience in a clinic setting.
- Team leadership facilitation and management experience.
- Equivalent combination of education and experience.
