High Intensity Care Coordinator

Job Description:

The High Intensity Care Coordinator also known as HICC, is a form of Targeted Case Management. The HICC position will hold a case load of 18 clients. In which they will be responsible for providing services and support which are guided by the needs of the youth and are developed through a wraparound planning process that is consistent with Systems of Care principles that result in an individualized and flexible care for the youth and family. The HICC position is designed to facilitate a collaborative relationship among a youth with SED, their family, and involve children serving systems to support the caregiving in meeting the youth's needs. The HICC facilitates the development of a care planning team comprised of both formal and natural supports who assist the family in the identifying goals and developing an individualized care plan and risk management safety plan. This position will conduct care planning meetings, along with coordinating and communicating with members of the care planning team to ensure the implementation of the care plan.

Requirements:
College degree required. Preferrably in Human Science.

• Previous experience in healthcare or related field preferred.
• Must possess excellent communication skills, both verbal and written.
• Must be able to adapt to changing priorities and work effectively with limited supervision.

Job Type: Full-time

Pay: $12.01 - $14.00 per hour

Expected hours: No less than 40 per week
