Job Description
TITLE:Help Me Grow Home Visitor
SUPERVISOR: Home Visiting Contract Manager & Supervisor, Help Me Grow
FUNCTION: Early Childhood/Family-Community Partnerships
JOB LOCATION: Stark County Educational Service Center
DATE: January 2024
JOB OBJECTIVE: To work in partnership with families to promote healthy pregnancies, improve child health/development/school readiness, improve parent confidence and competence, and increase family connectedness to community and social supports. Deliver the following components to all families: an evidence-based parenting education curriculum, ongoing screenings and assessments of the child(ren) and family, family-specific need-based referrals and resource linkages, and transitioning of child(ren) to a development-enhancing program/early care and education provider by age three.
JOB REQUIREMENTS:
  1. Minimum of an Associate's Degree in a field of study relating to child and family studies, child life, early childhood education, social services, child development, and nursing/other applicable healthcare field.
  2. Preferred a Baccalaureate in Social Work, Education with focus on Early Childhood, or nursing.
  3. Preferred licensure: LSW, Professional Teaching License in Early Childhood Education, or RN
  4. At least two years' experience working with families and children prenatal to age three, including the ability to establish trusting relationships.
  5. Knowledge of infant and child development and experience using child development assessment tools.
  6. Knowledge of community resources.
  7. Home visiting experience is preferred.
  8. Open to reflective practice (i.e. has capacity for introspection, communicates awareness of self in relation to others, and recognizes value of supervision.
  9. Data entry experience.
  10. Valid State of Ohio driver's license and proof of insurance.
  11. Acceptance of individual differences.
  12. Experience and willingness to work with the culturally diverse populations within Stark County.
  13. Experience in Infant Mental health (Infant Mental Health endorsement level I or II preferred)

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:
  1. Determine family's eligibility to receive Help Me Grow Home Visiting services.
  2. Assure that every family has a completed family-directed plan with at least one family goal as it relates to any of the four goals of the Help Me Grow Home Visiting program within mandated timelines.
  3. Assure that parents have been informed of their rights.
  4. Assure that developmental screenings of the child(ren) have been completed. If a delay is suspected, then a referral for a developmental evaluation must be made with the consent of the parent.
  5. If appropriate, referral and coordination of developmental evaluation and ongoing assessment in all five developmental domains, the family assessment, and hearing/vision/nutrition screenings to determine dual eligibility/re-determination for Part C services.
  6. Complete required tools used for parent/home assessment.
  7. Facilitate and participate in the development, implementation, review, and monitoring of the Family Plan and their timelines.
  8. Facilitate development of family outcomes.
  9. Parent education using the Growing Great Kids curriculum.
  10. Assist parents in identifying, establishing, and communicating with a medical/health home.
  11. Identify and connect families to specific need-based resources and providers.
  12. Inform families of the availability of advocacy services.
  13. Coordinate with medical and health providers.
  14. Coordinate transition of child(ren) to a development-enhancing program/early care and education provider by age three.
  15. Responsible for maintaining appropriate documentation for the client record (hard copy and electronic data systems) according to ODH and local rules/policies. Using this documentation, will work with Home Visiting Supervisor to assure accurate and timely Medicaid billing.
  16. Communicates and cooperates effectively, professionally, and respectfully with families/schools/professionals, and all others involved in the Help Me Grow system.
  17. Contributes to team effort by integrating individual responsibility with other team member responsibilities.
  18. Demonstrates commitment to continuous learning and self-improvement by reading and attending such meetings, conventions, and conferences as are necessary to keep abreast of the latest Help Me Grow developments and trends.

OTHER DUTIES AND RESPONSIBILITIES:

  5. Ensuring strict confidentiality regarding matters involving children, families, and staff.
  1. Complies with the policies and regulations of the county Superintendent of Schools, and the mandated state Help Me Grow Rules.
  2. Performs such other duties as may be assigned by the Home Visiting Contract Manager & Supervisor and/or the county superintendent of schools or his/her designee.
  3. Obtains and maintains State of Ohio Home Visitor credentials according to state Help Me Grow mandate.
  4. Evening and weekend hours may be required.

The job description shall become a part of the evaluation of the employee and shall be subject to an annual review of content and duties.
