This approach includes both crisis interventions, such as emergency shelter and family reunification services, as well as ongoing support to build family resiliency, including counseling, education, and parenting classes. ESSENTIAL FUNCTIONS

  • Completing all enrollment procedures with targeted families. This will include the organization and collection of documentation related to the eligibility of up to 42 families.
  • Participating in recruitment activities when appropriate.
  • Completing case management with families by assisting them with community resources and referrals to meet their needs.
  • Providing basic care management to families and accompaniment of parents to appropriate services or agencies when necessary.
  • Completing all required forms, case note documentation and monthly updates for assigned caseload.
  • Completing additional assignments, tasks, and reports as directed.
  • Planning and conducting weekly 90- minute child development home visits and monthly socialization groups for children ages 0-3 on the home based caseload that reflects the goals of children and families.
  • Coaching and mentoring of parents and developing reasonable steps and strategies within the context of their strengths and needs to help them realize their goals, taking into consideration the priority of each family and the outcome desired.
  • Coordinating family care with Child Protective Services, Department of Economic Security, chemical dependency programs, hospitals and doctors, schools and other relevant agencies.
  • Models Center’s expectation for sensitivity and competency relating to diversity and cultural awareness.

EDUCATION/EXPERIENCE

  • Associates Degree in Early Childhood Education, Social Work or related Field.
  • 1+ year of related experience in infant / toddler education and / or family case management preferred.
  • Highly proficient with Microsoft Office Suite and comfortable with technology in performing job duties.
  • Home visitation experience with a focus on child development a plus.
  • Must demonstrate excellent written and verbal communication skills.
  • Bilingual skills preferred
  • Must also be able to successfully complete a background check and drug screen.
  • Must be able to obtain Class One Fingerprint Clearance Card immediately upon hire (if there are any uncertainties whether an applicant may have to go through an appeal process: they are encouraged to complete that process prior to applying, as an invalid card will result in termination of employment).
  • Must be at least 21 years of age
