The Center for Prevention and Early Intervention Policy

Responsibilities


Plans and conducts educational sessions during weekly home visits with pregnant women, infants, and toddlers. Sessions will be based upon assessment of child and family needs, utilizing Early Head Start protocols, policies and procedures.

Develops written child and family case documentation including: home visit notes, child assessment results, child and family goals progress, and parent-child interactions through timely entries into the program database and quarterly reporting.

Provides prenatal education for expectant families using the Partners for Healthy Baby curriculum to promote healthy birth outcomes and ensure linkages with pre and postnatal care providers.

Plans and facilitates Group Socialization experiences twice monthly for children and families served in the home, including encouragement of group participation from families.

Assists in arranging regular medical appointments and gathering medical documentation for families enrolled in the program.

Conducts accurate screening and ongoing assessment of young children's developmental progress utilizing the Ages and Stages Developmental Questionnaire (ASQ) and the Early Learning Accomplishment Profile (ELAP).

Provides referrals and assists families in identifying and utilizing community resources, while encouraging and promoting each families empowerment and self-sufficiency.

Qualifications


Bachelor's degree and two years of experience; or high school diploma and six years of experience. (Note a combination of post high-school education equal to six years.)
A valid Florida or Georgia driver's license or the ability to obtain prior to hire.
Completion of all requirements necessary for work in a healthcare environment in accordance with state laws, Department of Health, OSHA, and/or CDC as performed by a qualified physician.
Physical examination completed by a qualified healthcare provider/physician.

Preferred Qualifications


Bachelor's degree in Social Work is preferred.
