Tidewater region, looking specifically at hiring for coverage on the eastern shore of VA, with potential for someone in Exmore, VA up to Chincoteaque, VA. Must be located in the counties of Accomack or Northampton.
Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively
manage healthcare costs and improve healthcare program/operational efficiency involving clinical issues
Nurse Case Manager is responsible for telephonically and/or face to face assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness.

2-3 years Clinical practice experience, e.g., hospital setting, alternative care setting such as home health or ambulatory care required.
2 years Healthcare and/or managed care industry experience.
Case Management experience required-- Position requires proficiency with computer skills which includes navigating multiple systems and keyboarding
