This is a Local Union 509 full-time, non-exempt (35 hour/week) position. We offer a hybrid remote schedule (1 day/week in the office) and the option of a flexible 4-day work week. Our office is located in Malden, MA with ample free parking!

Case Manager Compensation: Starting annual salary is $47,274.86. Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin. As the Case Manager you will be responsible for assessing, reassessing and monitoring consumers for need and eligibility for in-home services, recommending/approving service plans, providing information on available resources, and coordinating in-home/community services.

What you'll be Responsible for as the Case Manager:

Essential functions of the position are below. Additional duties may be assigned as required.

• Conduct initial and on-going assessment of consumers' health and functional status.
• Conduct visits as required in various settings including consumers' homes, community settings, and/or medical facilities.
• Participate in development of consumers' care plans, including determining appropriateness and need for in-home/community-based services.
• Participate as part of consumer's primary care team including, attending team meetings, trainings, and joint home visits.
• Arrange, coordinate, and oversee in-home and community-based services for consumers.
• Monitor consumer's health status, in conjunction with care team, to assess for effectiveness of current services, need for additional services, and/or need for long-term placement.
• Assist consumers with MassHealth redeterminations and other benefits paperwork, as requested and appropriate.
• Maintain consumer records in accordance with standards.
• Compile and submit required reports.

Case Manager Qualifications:

• A Bachelor's degree in Social Work or a related field.
• Social Work license preferred.
• Knowledge of and experience working with older adults in a community-based setting preferred.
• Strong interviewing and assessment skills.
• Strong interpersonal and organizational skills.
• Excellent written and verbal communication skills.
• Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches
• Private transportation.
