WORK DESCRIPTION:
Provides individual services to adult, substance-using patients by following evidence-based practices; assists patients in linking to recovery support services, health care and other wraparound supports; and develops individual Service Plans that help patients enhance self-efficacy, maintain engagement in medical care, maintain medication adherence, and reduce HIV risk and transmission behaviors.
ESSENTIAL FUNCTIONS:
Carries caseload of HIV positive or at-risk patients at Haymarket Center
Assist patient in receiving support services such as healthcare, housing, and public benefits, HIV and hepatitis treatment, and PrEP/PEP services
Communicate with medical staff when screenings or tests indicate high risk of HIV, HCV, or other infectious disease
Provide education about risks, symptoms, preventive practices, and resources related to HIV, HCV, and other infectious disease
Provide linkage-to-care activities for HIV, HCV, and other infectious diseases
Maintain advocate position for patient
Complete documentation and reports in Electronic Care Record in a timely manner
Facilitate didactic and therapeutic groups when required
Develop appropriate community resources such as case management, mental health services, substance abuse treatment, food, housing, and public benefits among others
Provide education and support for engagement in medical care, medication adherence, and HIV risk reduction techniques.
Support patient through navigation of health care
Collaborate with other service providers to create network of providers to ensure patient linkage to services and recruit participants for Haymarket Center programs and service
Attend programmatic meetings, trainings, and supervisions
Follow agency regulations regarding vehicle use
Other duties as required for effective teamwork or as assigned by the Project Coordinator
Requirements
EDUCATION REQUIRED:
A Bachelors degree in Public Health or health related field preferred.
EXPERIENCE AND SKILLS:
Knowledge of HIV, Hepatitis C, STIs, PrEP, and Hepatitis C highly preferred;
Experience working in a similiar role;
Experience working with targeted population mandatory;
Bilingual (English/Spanish or English/Polish)
Proficiency in reading and writing required;
Able to work effectively on a team required;
General computer literacy is mandatory;
Previous experience working with an EHR highly preferred;
Understanding of HIPPAA, 42CFR, AIDS Confidentiality Act required;
Must maintain a valid Illinois Driver's License and eligibility for agency insurance coverage required.
