Health Coach Fountain Life
We are seeking a qualified Health Coach to join our team at Fountain Life to work as part of an integrated care team including nurses and member experience coordinators and led by a concierge physician. The Health Coach will work one on one with Fountain Life Apex members to help them implement their care plans. They will ensure each member feels cared for and will work closely with the other members of the care team to provide ongoing touchpoints with the goal of helping each client reach their personal goals and optimize their health.
Key Responsibilities:
- Establish rapport and empower clients seeking to enhance well-being through self-directed, lasting changes, aligned with their values.
- Influence client behavior by educating clients on the importance of lifestyle changes for improving health outcomes.
- Support clients by translating their recommendations into practical and actionable plans that incorporate functional medicine principles.
- Provide client support and accountability with regular touchpoints through video calls, telephone, and electronic messaging or email.
- Excellent follow-up and time management skills, ensuring clients are being guided through their journey at Fountain Life.
- Engage and delight clients by anticipating their needs and going above and beyond to solve problems.
- Manage client progress and communication with appropriate documentation in clients' charts.
- Collaborate with other healthcare professionals and team members to provide integrated, complete care.
- Keep up to date with the latest research and developments in functional medicine and Health Coaching. Regular check-in's and follow-up on action plans set with Health Coach.
- Help clients identify barriers to implementing lifestyle recommendations.
- Address any questions or concerns.
- Coordinate supplement and medication refills.
- Ensure clients are tolerating all medications/supplements well
- Reinforce nutrition agreements.
Qualifications:
- Training from the Functional Medicine Coaching Academy is preferred.
- Strong interpersonal skills with the ability to establish rapport and empower clients.
- Knowledge of lifestyle changes for improving health outcomes.
- Experience in providing client support and accountability.
- Excellent follow-up and time management skills.
- Strong problem-solving skills.
- Ability to collaborate with other healthcare professionals and team members.
- Knowledge of the latest research and developments in functional medicine and Health Coaching.
