We have an opening for a part-time contracted Health Coach on the Brook Health Coach team. The Coaching team focuses on providing excellent motivational interviewing and coaching to help members make healthy daily decisions and lifestyle changes. Whether they are looking to prevent diabetes or are just looking to make healthier choices, Brook Coaches provide daily guidance on the Brook app. We have been an app-based digital health company for over four years, serving over 30,000 people.
This position is part-time, work from home, remote work. The expected hours/week is around 25.
The successful candidate will be able to:
- Be customer-centric in your approach
- Work remotely with self-discipline to meet assigned performance goals
- Manage large caseloads using proprietary web-based tools
- Be organized and own the responsibility for the success of members on our Health Coach platforms
- Be a quick learner of new technologies and tools
- Bring previous experience with coaching for general wellness and diabetes prevention
- Be completely comfortable and competent with using common administrative technologies (e.g. Zoom, Slack, Office or equivalent) and app based platforms
- View real-time data from members and provide proactive feedback in the form of regularly scheduled outreach to these members
- Experience educating and coaching those living with chronic conditions through behavior change to sustain healthy lifestyle habits
Preferred:
- Experience delivering coaching via digital messaging and telephone
- Certification from the CDC to deliver the Diabetes Prevention Program
- CHES or NBHWC credential
Once onboard, this person will be cross trained to work in many areas of our digital health platform supporting users in app and via telephone calls, in our Remote Care and Diabetes Prevention Programs.
