Job Description
A bit about this role:
This position represents an amazing opportunity for an ambitious, clinically focused Geriatrician ready to help lead a virtual geriatric service with in-home support and an interdisciplinary team. The Geriatrician Team Leader is the true champion of our patients’ care, and builds trusting patient relationships and empowers patients to live healthy, dignified, independent lives. The Geriatrician also plays a critical role in teaching their team age-friendly concepts. Excellence in this role will entail working with leadership at Devoted Medical to continuously improve performance of this program around a high clinical standard as we scale, based on best-practices and use patterns identified throughout the growth phase of the program. In partnership with Community Geriatrics leadership, the Geriatrician Team Leader will take ownership of clinical performance, cultural alignment, people management, and clinical operations.

The Community Geriatrics program is built on two pillars. The first is to develop a model guided by the principle of
geriatrics
as “the experts in complexity and all that comes with it, including uncertainty, tradeoffs, interdisciplinary teams, multiple coexisting conditions, patient goal-driven care.” The second is the burgeoning field of community paramedicine, which transforms paramedical professionals (EMTs and paramedics) into agents of home-based, relational chronic disease management. Based on these two pillars, we are developing a program that focuses on home-based care for high-cost, high-needs patients through an agile multi-disciplinary team. The Geriatrician Team Leader will be delivering age-friendly care grounded in the 5 M’s framework for patients enrolled in the program.

Your Responsibilities and impact will include:
  • Deliver exceptional care to a cohort of complex patients across multiple geographies, leveraging video telehealth visits and a multidisciplinary team consisting of nurse case managers, and field-based geriatric technicians.
  • Clinically manage the patients, with a strong focus on complex geriatric conditions (utilizing the 5M framework) such as polypharmacy, falls and dementia.
  • Engage patients, patients’ caregivers/family, existing provider network & other Devoted programs to provide cohesive, comprehensive care experiences & optimal clinical outcomes. This will include working closely with patients’ non-Devoted PCPs & specialists, in addition to Devoted’s many specialized clinical & psychosocial teams, such as pharmacy, disease management & education, social work & palliative teams.
  • Teach core Geriatrics principles to the other disciplines on the team, ensuring that these principles are the foundation of the team’s clinical model
  • Utilize our home grown electronic health information system (Orinoco) for visits while also providing feedback on how to improve the interface.
  • Steward our patient-centric and open clinical culture to our patients and colleagues.

Required skills and experience:
  • A fellowship-trained, and board-certified Geriatrician with at least 3 years clinical experience.
  • Telehealth experience preferred.
  • Experience managing other clinicians.
  • Team player, continuous improvement mentality and can-do attitude.

Licensure and Certification:
  • Multi-state licensure preferred or willingness to obtain additional licensure as requested.
Salary range: $220 - 300K annually
