Queensbridge Health Advocacy Partners Program (HAPP):Full-Time Supervisor (6-month project)

PROGRAM OVERVIEW: The NYC Public Health Corps (PHC) is a citywide investment in and commitment to the public health workforce and just recovery from COVID-19 with and for communities that have been disproportionately impacted. Following several years of COVID-19 specific work, the next phase of PHC aims to focus on chronic disease prevention and management through decreasing inequities in health and social outcomes for residents of public housing.

The Health Advocacy Partners Program, a component of PHC, will drive equitable pandemic recovery in priority public housing communities that have experienced longstanding health inequities. The program is funding community-based organizations to build up and utilize a workforce of community health workers (CHWs) to deeply engage with targeted NYCHA neighborhoods in preventing and managing chronic conditions like asthma, hypertension and diabetes. The program will work with NYCHA residents to help them better manage their chronic illnesses through:

· § One-on-one health coaching sessions

· § Individual and group-level educational workshops

· § Assistance in acquiring and navigating health insurance

· § Referrals to clinical and social support services in the community

· § Advocacy and community organizing

HAPP community partners will be working in the following neighborhoods and NYCHA developments:

Queens Long Island City Queensbridge (north and south)

Position Overview: Responsible to conduct comprehensive assessment and regular reassessments. Guide service planning and updates. Routinely communicate and collaborate with Community Health Workers and other staff who are engaged with the client. Help lead and guide a team of 10 Community Health Workers to ensure services are provided to all clients assigned to meet clients’ needs for better health outcome.

The major duties of the position include, but are not limited to:

  • Report to Program Manager by providing day-to-day supervision to Community Health Workers assigned by weekly schedule monitoring, chart review, face to face case discussion, and performance

reviews.

  • Train and coach, the assigned staff and/or new team members of workflow and requirement

changes), and performance expectations and excel in their performance.

  • Responsible for ensuring the team meets services quota established and contractual

requirement: if circumstances cause changes, plan and implement to meet target goal.

  • Create and maintaining up to date logs and other required reporting methods.
  • Maintain accurate records on the services and activities such as attendance, registration, client’s

interactions and other required documentation according to the protocols and standards of

HAPP and DOHMH within required timeframe.

  • Working as liaison between program manager and the CHW team for the implementation of client health plans, events and offerings.
  • Collaborates with the NYCHA staff, tenant association and other stakeholders to manage care coordination resources.
  • Facilitates interdisciplinary conversation and planning.
  • Served as back-up for the assigned staff as needed.

The recommended qualifications for this position are:

· BA/BS in social science, psychology, social work, health or equivalent field with three (3) years of qualifying experiences in case management, health coaching, and/or care coordination required. An advanced degree in one of the qualifying fields may be substituted for one (1) year of qualifying experience may be considered.

  • One (1) year of supervisory experience to paid staff preferred.
  • Qualifying experience includes verifiable work with the target population: individuals with substance use disorders (SUDs), serious Mental Illness, developmental disabilities, individuals with high risk of or living with HIV/AIDS or other chronic diseases, LGBT, Asians and Pacific Islanders, and immigrants.
  • Strong socio-cultural identification with the target population or experience in working with target population strongly preferred.
  • Appreciation of and commitment to CMS mission and values.
  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint).
  • Excellent speaking and writing in English required.
  • Excellent customer service skills to assist in resolving member complaints.
  • Ability to work effectively in both a collaborative as well as individual setting.
  • Appreciation of and commitment to CMS’s mission and values.
  • Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing CMS.
  • Fluency in Spanish or one of Asian languages preferred.
  • Ability to work flexible hours including some evenings and weekends required.

Salary $65,000 (6 months)

Job Types: Full-time, Temporary
