Adventist Health Clear Lake is a 25-bed, critical access hospital serving Lake County with key service areas including 24-hour emergency care, imaging, intensive care, maternity, physical rehabilitation, pulmonary rehabilitation, surgery and 10 rural health clinics that provide behavioral health, cancer, dental, family medicine, OB/GYN, pediatrics, neurology, orthopedic and pain management to name a few. Job Summary:
Develops a wellness coaching relationship with clients and assisting the client through the process of actively working towards better health by providing support, encouragement, and education. Assesses health data, coaches members and coordinates lifestyle interventions to directly influence health outcomes. Provides a collaborative approach to health risks, and promotes optimal wellbeing in defined populations.

Job Requirements:
Education and Work Experience:
- Bachelor's Degree in exercise science or equivalent (some certifications will require Bachelor's): Required
- Master's Degree: Preferred
Department Specific License/Certifications:
- Certified Medical Exercise Specialist (CMES): Required
- Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
Essential Functions:
- Contacts participants that are eligible for the program for enrollment and provides necessary wellness coaching to reduce or eliminate high-risk behaviors. Encourages adoption of habits that are conducive to a higher quality of life. Supports operational aspects of the division.
- Develops and conducts onsite health-related wellness seminars and training courses for interested employees and family members.
- Develops and distributes health education materials to identified employees and family members.
- Develops a care plan for program participants including goals and plan of action. Coordinates care with other health-related vendors for optimal patient care.
- Performs other job-related duties as assigned.
