OVERALL JOB PURPOSE:

The primary responsibility of the Family Success Coach is to provide home and community-based services to families in need of support. Coaches are assigned to families of children and youth who previously experienced child maltreatment and are exiting foster care for a permanent placement (reunified, adoptive and guardianship families). This position will primarily provide post-care services to families who reunify or receive guardianship of a child from the child welfare system. In-home services will enhance caregiver competency, self-efficacy, and overall child well-being.


Scope:

The primary duty of the Family Success Coach is to provide ongoing support services including assessment, crisis intervention, service planning, goal setting, home visitation, counseling, parent education, case management, monitoring, community referrals, reporting, and skill reinforcement to youth and families following involvement in child welfare services. The caseload is estimated to fluctuate between twelve and eighteen families at a time. The intensity of services needed will vary based upon the unique situation of the family. The position is also responsible for appropriate and timely documentation and completion of forms according to case flow timelines and for evaluation purposes.


The Family Success Coach will provide services to families for up to twenty-four months. Services are based on the individual needs of the youth and family. The Family Success Coach will have the ability to adjust their work schedule based on the needs of the families served and must be available to work evenings and weekends hours, and is responsible for providing telephone availability to clients 24- hours per day, 7 days per week when necessary for crisis management.


Family Success Coaches are expected to travel within the communities served and must make initial face-to-face contact with the family within seven days of the receipt of the referral which is also documented.


JOB RESPONSIBILITIES:

PERCENT


1. Engagement, Assessment and Goal Planning - Recruit families for the Family Success Coach service and educate community and agency partners in counties served about the service. Build rapport and establish a consistent, professional relationship with the family being served. Work in conjunction with the family to complete a comprehensive assessment, develop a service plan and set goals that address placement stability, build caregiver self-efficacy, and support overall child and family wellbeing. Engage the family and deliver in-home services, crisis intervention, case management, and skill-building around problem-solving, self-advocacy, conflict resolution, and relapse planning. Review and update service plans and goals in accordance with service provisions or as needed. Service plans must be updated a minimum of annually. Maintain minimum caseload of twelve to eighteen families.


2. Skill Building and Reinforcement & Crisis Prevention/Intervention - Provide individualized skill-reinforcement and coaching to model appropriate parenting/family behaviors during home visitation based on Strengthening Families Parenting Model and needs identified in the goal plan or as situations arise. Provide 24 hour a day, 7 days per week telephone availability to assist families in weathering a crisis. Complete crisis prevention planning and skill building based on the families individual needs


3. Service Coordination - Advocate for the family's needs through community collaborations and connections. Provide information, make referrals, and facilitate introductions that connect families to appropriate community resources and informal supports that strengthen the family's self-efficacy. Build and maintain relationships with various community resources, inter-agency networks, and informal family supports.


4. Documentation - Complete appropriate and timely reports of family visits and interactions, complete documentation needed for intake and ongoing evaluation purposes, as well as other record-keeping and reporting, as required by agency policy or project guidelines (documentation is to be complete within 7 days of interaction with family); maintain confidentiality as required by statute/policy.


5. Professional Development - Participates in internal meetings and minimally obtains required training such as new employee orientation, Motivational Interviewing, Parenting Skill building, policy and procedure training, and other, in-service training, as required. Actively seeks out training that builds skills in child welfare and post-care, best-practice models and available community resources. Participate in ongoing supervision meetings. Maintain professional boundaries with families served.


6. Evaluation Activities - Complete initial assessment with families and conduct 6-month follow-up assessments. Follow all required activities to adhere to evaluation protocol. Provide all evaluation materials to supervisor.


7.Teamwork, Quality Improvement, and Cultural Sensitivity: Promotes the spirit of teamwork. Family Success Coach will demonstrate the ability to be a good colleague and will strive to continuously improve the quality of the agency and self. These competencies will be carried out in order to increase the efficiency of the program. The Family Success Coach accepts cultural and individual differences, upholds the vision and mission of the agency. The Family Success Coach is an active participant in all agency initiatives to achieve these goals.


Internal Contacts: All levels of CYDC management staff and support staff.


External Contacts: Clients and their families, referring agencies and other community professionals.


License or Certification: Valid SC driver's license with a good driving record.


Decision-Making Authority: Must possess the ability to make sound decisions weighing all of the factors for good judgment based on CYDC policies and procedures. Decisions must reflect good clinical skills as well as the best interests of the clients and families.


Major Challenges:

  1. Ability to work independently and manage workflow. Must possess adequate transportation with appropriate insurance coverage for transporting parents and children.
  2. Ability to work with parents and children from diverse populations and placement types and who may have challenging behaviors.
  3. Ability to assess needs and ensure the safety of the child during post-care.
  4. Consistent review of service plans and modification as needed.
  5. Possess warm, empathic, and non-judgmental relationship-building skills and communication style to establish a trusting relationship within a family.
  6. Maintain a caring relationship with clients while allowing for independent growth.
  7. Demonstrate successful parenting with effective behavior management skills or have extensive experience in the childcare field.
  8. Possess a knowledge and understanding of basic parenting skills and family dynamics, child welfare laws, and models for teaching skill-building.
  9. Considerable knowledge of community resources and ability to maintain community partnerships to meet the needs of families involved with the program.
  10. Ability to be a positive role model to families and maintain appropriate boundaries.
  11. Exhibit organizational and writing ability for data collection and to facilitate the maintenance of accurate and complete records.
  12. Have the ability to be a staff team player working together to achieve the goals of the program and of the agency.

Functional Knowledge:


Type of Knowledge Required and or Skills Required: Must maintain professional integrity and ethics and regular and predictable attendance. Must possess the knowledge and skills necessary to work with a diverse population. Must have the skills to actively engage other professionals as well as excellent communication skills and the ability to develop well-rounded, behavioral interventions. Must demonstrate the ability to react to change productively and handle other essential tasks or responsibilities as assigned. Must display excellent organization and time management skills.


Education and Specific Training: Minimum qualifications include: (1) Master's degree in a field related to social work and two years of social work experience working with the population being served; or (2) Holder of an active license in a field related to social work and one year of experience degree in a field related to social work.


Experience: Two years of social work experience working with the population being served.
