Description
The full performance level class works under general supervision & requires considerable knowledge of social work, departmental regulations, policies, procedures; state and federal regulations; crisis intervention, counseling and interviewing of clients activities related to the assigned social service program as a part of the delivery of the program to participants/clients. This class is utilized in multiple departments and is involved with service delivery to a range of client populations.
Normal Working Hours: Monday - Friday; 7:30am - 4:00pm. Some evenings and weekend hours required.

Duties/Responsibilities
Under the direction of the Family Stability Coordinator provide case management and support to families and their children engaged in service coordination/High Fidelity Wrap-Around (HFWA) and at risk for out-of-home placement, including establishing and maintaining effective relationships with families, conducting home visits/school visits; providing linkage to needed services, identifying and assisting in remediation of barriers to service access, and follow-up to ensure services are accessed/received and family remains engaged; using an evidence-based curriculum engage in parent coaching/role modeling of positive, effective parenting skills.
Attend and participate in wrap-around/team meetings, agency reviews, conferences, and job-related trainings.
Compile, document and report case management information and data; prepare statistical reports; establish and maintain effective working relationships with representatives of various social service agencies, court officials, law enforcement, etc.
Perform miscellaneous tasks; assist, as requested, with activities to ensure on-going unit and department operations (i.e. phone/front desk coverage); respond to special requests for assistance related to unit's/departments objectives and role; participates in events and activities, as required and/or approved, related to professional development (i.e. participate in unit and department meetings, supervision, in-service, training).
Minimum Qualifications
Completion of core course work for Bachelor's Degree in Social Work or closely related field; 18 mos. experience in social services; valid Ohio Driver's License.
- Or equivalent of Minimum Class Qualifications for Employment noted above may be substituted for the education and/or experience required. Education and Experience may not be substituted for mandated licensure/certification.

UNUSUAL WORKING CONDITIONS:
Predictable and regular attendance is required.
Travel may be required.
May work rotating shifts (e.g. evenings) with rotating days off, weekends and holidays.
Stooping, reaching, standing, walking, pushing, pulling, lifting, grasping, feeling, talking, hearing, seeing, and repetitive motions.
Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or negligible amount of force constantly to move objects.
