About the role
The role of the Family Care Coordinator is to support the LyN patients and children with any and all non-related treatment necessities and matters to ensure the highest quality of patient care is sustained, and that needs are met on a daily basis. This role will support the mission, vision, values and goals of Casa Esperanza as an organization, and ensure smooth and effective functioning of the LyN residential treatment program.
Essential Functions and Responsibilities:
- Responsible for all patient and children crisis intervention management
- Manage All Patient DCF Cases & Matters: Adhere to the processes put in place, such as sending over toxicology screens twice per week, as well as daily/weekly updates, meetings, phone conferences, zooms, coordination of reunification plans, coordinating weekly child visits, collateral contacts, etc.
- Management, coordination and follow through with patient medication refills; coordinating with prescribers, as well as the pharmacies to ensure timely distribution of medication refills and pick ups.
- Collaborate with the integrated care team, and all departments, to ensure that all patients and children's needs are met in a timely manner.
- Management, Coordination of, confirmation of all internal and external patient appointments (FUOP, external medical, dental, psychological, etc. providers for both patients and children)
- Coordinate appropriate transportation to and from any and all patient and children's appointments; ubers, public transportation, etc.
- Coordinate and collaborate with colleagues and collateral contacts re: ATR, Housing, DTA, Employment Coordination, WIC, Daycare Vouchers, Feeding Consultants, Parenting Classes, DV Courses and any and all Mother's Supports including family support services.
- Manage and Coordinate any and all Patient legal obligations, appointments and responsibilities; parole, probation, and court dates (in coordination with our ORP program coordinator)
- Coordinate all elementary school logistics (zoom and on site) and school necessities (school-aged children)
- In addition, all other non-treatment related matters that occur on a daily basis
- Approval, Confirmation and Coordination of all pick-ups and drop offs of children
- Work to establish support groups for collaterals and families incolved in the patient care plan

- Participate in MDT meetings to coordinate with internal staff regarding services delivered to residents through the Familias Unidas Outpatient Clinic and ensure service plan goals align with treatment plan goals
- Facilitate weekly treatment/recovery groups
- Participate in weekly supervision meetings
- Participate in weekly clinical team meetings
- Other duties as assigned per SAMHSA grant requirements.
Skills necessary for the position:
- Ability to work as a team member
- Ability to work independently
- Knowledge of substance abuse field, including the AA/NA 12 Step philosophy
- Strong Crisis management/intervention skills
- Knowledge of appropriate community resources
- Strong verbal and written communication skills
- Ability to communicate with clients and referral sources in a professional manner
- Strong interpersonal skills and customer service skills
- Knowledge of computer software, including Microsoft Word and Excel
- Valid Massachusetts Driver's License
- Ability to meet the physical and training requirements of the position.
Experience, Education & Training necessary for the position:
- Experience working in an advocacy role for children and families
- Experience with homelessness, mental illness, domestic violence, HIV/AIDS, and child case management
- Experience with motivational interviewing and trauma-informed care, preferred.
- Bachelor"s level education experience in related field around family planning, human services, early education
- minimum 2 years experience working with family systems
- Bilingual Spanish/English required with intimate knowledge of the cultural social, economic, health, and substance abuse factors.
- CPR Certified
