The role you’ll contribute:

The Executive Director (ED) of System Care Management for Central Florida Division-South Region is responsible for strategic planning and outcomes related to patient throughput via care management and transition planning to post-acute settings. The ED has executive oversight for the care management model and strategy, education and regulatory alignment, referral management and complex transitions including the complex care team and readmission strategy. The ED will develop and execute operational strategies built to ensure care management strategy is aligned with regulatory requirements, organizational goals and forward thinking aimed at efficiency and viability. The ED will develop strong relationships with PHD and community post-acute partners in order to drive efficient care and discharge plans. The ED provides leadership and direction to campus operators related to the translation of strategy into care management initiatives and clinical priorities. Works collaboratively with physician leadership to lead strategy related to care management ensuring stakeholder inclusion and alignment.

Scope of Responsibility

  • Oversight of Care Management. Direct reporting relationship with the system care management operations team including care management model and strategy, the education/regulatory team, complex care team, referral management team, transition specialist/readmission strategy team and the post-acute contract billing team. Indirect reporting relationship with the campus care management leadership.
  • Monitors all applicable performance metrics related to the management of the patient’s hospital stay and post discharge planning including but not limited to LOS and Readmissions.
  • Oversight of system level contractual agreement for Care Management related programs including post-acute facility agreements and single patient agreements as applicable.
  • Develops and implements strategy and actions plans in alignment with the organization’s strategic planning process. Ensures strategies developed and deployed for campus level accountability related to performance metrics. Ensures alignment with regulatory guidelines and organization goals. Adjusts strategy as indicated by performance metrics and/or organization need.
  • Assures evidence-based practice and clinical best practice care standards, outcomes and metrics are established and held accountable at each campus. Establishes and improves systems and processes to support continuous learning and improvement based on quantitative and qualitative information regarding outcomes and quality.
  • Develops a high performing culture by keeping leadership engaged and constant communication with physicians and clinical leadership teams within areas of oversight.
  • Strategically evaluates and improves policies, procedures, and evidence-based practices consistent with the organization’s goals and all regulatory requirements and standards.
  • Collaborates with physician leadership to lead the clinical team in achieving Connected, Wholistic, Affordable, Viable and Exceptional (CWAVE) care with the best possible clinical outcomes and patient satisfaction by providing best practice quality care in a safe environment.
  • Participates in the patient safety evaluation system and maintains the integrity of the patient safety work product according to established policies.
  • Promotes collaboration with Nursing/Clinical/Care Management leadership team, physician leadership team, medical staff and affiliated services in an effort to advance the clinical practice and the efficient delivery of care.
  • Maintain organizational compliance with all applicable state and federal regulatory requirements relative to the delivery of care.
  • Plans and develops a complete succession plan for rollup up to and including own position. Identifies high performing staff and works with Leadership Development and Talent Management to groom staff to meet succession plan timelines and curriculum. Actively mentors others into the leadership roles throughout the timeline.

Duties:

  • Develops and oversees programs that monitor resource utilization, patient care coordination, clinical document integrity, denial avoidance and optimal patient care transitions for the region.
  • Creates and oversees implementation of organizational strategies across the region to accomplish utilization and length of stay goals, including organizing and facilitating interdepartmental teams, while ensuring efficient coordination of clinical care and appropriate social needs.
  • Provide oversight and facilitates collaborative relationships with CMOs, Physician leaders, Coding, Care Management, Patient Financial Services and other functions to optimize the clinical documentation integrity across the region.
  • Serves as a subject matter expert for Care Management, Transitions of Care and Care Coordination.
  • Consults with the regional and campus Executive Teams regarding Care Management, Social Services, Denial Management, Care Coordination, Transitions of Care and Clinical Documentation Integrity operations. Informs and updates local facility CMOs and Executive Teams on issues that impact the strategic goals and objectives of the facility.
  • Responsible for regional oversight and direction for the implementation and sustainment of Care Management Best Practice initiatives and standard work.
  • Ensure compliance with all federal and state regulations including, but not limited to Medicare, TJC and licensure requirements.
  • Evaluates the department/organization’s compliance with TJC, CMS and other regulatory standards and develops, communicates, and enforces departmental policies and procedures consistent with organizational, governmental and TJC standards.
  • Serves as a transformational leader and develops and monitors strategic plans and implementation for driving care coordination in a manner that is both cost effective and centered on the consumer.
  • Communicates and collaborations, is visible and accessible to leaders, physicians and staff across the region, division and corporate.
  • Oversees campus and regional directors to ensures technical and managerial competence of departmental leadership compliance with organizational values (TWCE) by the leadership team.
  • Demonstrates and leads service management philosophy in departmental and interdepartmental environment.
  • Holds campus and regional directors accountable to expectations.
  • Effectively communicates with and influences executive stakeholders.
  • Maintains professional affiliations and enhances professional growth and development to keep up with the latest trends in healthcare.

KNOWLEDGE AND SKILLS REQUIRED:

  • Knowledge of clinical practices and processes, legal and regulatory requirements and mandates, and the ability to gather and evaluate data and outcome results to use in planning hospital operations, budgets, and process improvement.
  • Outstanding written and verbal communication skills. Able to effectively express ideas and views through public speaking, presentations, reports, and professional correspondence.
  • Exceptional people management, leadership skills, and the capacity to relate to people in a manner that wins confidence and establishes support.
  • Strategic vision and thinking. Ability to position the organization for the future, looking beyond the present situation to conceptualize key trends and identify changing market demands.

  • Strong organizational abilities and the flexibility to adjust to changing conditions and the various details of the position.
  • Proficient computer skills, particularly with Microsoft Office suite.

Qualifications

The expertise and experiences you'll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

  • Master’s degree in Social Work
