Job Description
Engagement and Enrollment Specialist/Care Manager engages and enrolls eligible individuals in Care Management services and assists with member retention needs. The Enrollment Specialist conducts telephonic, written, and face-to-face outreach and retention services to coordinate care for Health Home Care Management eligible members. The Enrollment Specialist works closely with the Care Management team to identify, locate, engage, and enroll members eligible for Health Home services. The Enrollment Specialist assists the team by providing information about community resources that can support member wellness goals.
MAJOR RESPONSIBILITIES / ACTIVITIES:
- Provide telephonic, written and face to face engagement and enrollment services for each Health Home eligible individual to facilitate Health Home enrollment and retention.
- Utilize internet databases, health record systems, and other resources to explore individual eligibility and contact information.
- Track and document all outreach attempts and enrollment in designated Care Management Portal (Medicaid Analytics Performance Portal; MAPP) and Electronic Health Records (EHRs) as required.
- Complete all enrollment documentation/consents from the individual or legal guardian as required in a timely fashion.
- Track outreach and engagement of Lead Health Home and other internal/external referrals as required.
- Represent the agency at various venues to promote agency services.
- Responsible for maintaining and tracking the progress of potential members, their assignments, and their outcomes. Maintain contact with the members from enrollment until assignment to a care manager.
- Complete initial assessments (Child and Adolescent Needs and Strengths; CANS) to determine the individual’s most appropriate level of care management.
- Complete initial Crisis Plan, Comprehensive and Care Plan.
- Meet client contact requirements and conduct face-to-face visits on a consistent schedule as
per the mandates of the member’s acuity level (high, medium, low).
- Meet Care Management documentation requirements in a timely and accurate manner by effectively utilizing designated Care Management Portal (Medicaid Analytics Performance Portal; MAPP) and Electronic Health Records (EHRs) as needed.
- Effectively communicate and share information with the individual, their families and other caregivers with appropriate consideration for language, literacy, and cultural preferences.
- Conduct initial care plan meeting/conference and serves as an interdisciplinary team member to effectively provide/coordinate comprehensive and holistic care.
- Conduct and facilitate transfer meetings and introductions to new care management staff as appropriate.
- In the event of hospital admissions, actively engage in the discharge planning process ensuring that the patient has all recommended post discharge services in place prior to discharge.
- Attend and participate in ongoing staff development trainings to enhance skills needed to effectively meet the demands of the Engagement and Enrollment Specialist/Care Manager position.
- All other duties as required.
SALARY RANGE:
Bachelors - $26.37/hr - $28.57/hr
Bachelors (Bilingual)- $27.47/hr - $29.67/hr
Masters - $28.02/hr - $31.59/hr
Masters (Bilingual) - $29.12/hr - $32.69/hr
Masters (Bilingual & License) - $35.43/hr
MINIMUM REQUIREMENTS
Education and/or credential requirements are determined by children’s acuity level and require one or more of the following:
- Bachelors of Arts or Science with two years’ experience or Master’s Degree in related field (License preferred)
- Bilingual (Spanish Required)
The candidate must possess the following Experience:
- Relevant expertise and experience in serving children and families in child welfare, developmental disabilities, mental health, healthcare and/or other systems as well as those receiving preventive services.
- Experience providing Care Management/Care Coordination or service coordination and information, linkages, and referrals for community-based services.
In addition, the candidate must demonstrate the following Competencies:
- Excellent writing, communication, and organizational skills
- A passion for serving children and families with special needs
- Ability to travel to community-based agency offices, participant’s communities, and homes within assigned catchment area
- Ability to receive feedback to professionally grow and/or improve
- Proven self-management abilities, including meeting deadlines, ensuring compliance with agency policy and procedures, and overseeing complete and timely maintenance of agency records, in accordance with contractual requirements.
