KHS reasonably expects to pay starting compensation for the position of Enhanced Care Management Justice Involved Liaison in the range of $41.08 - $53.27 hourly.

PRIMARY PURPOSE:
Under the direction of the Enhanced Care Management (ECM) Leadership, the Kern Health Systems (KHS) ECM Justice Involved (JI) Liaison will serve as the KHS lead point of contact for Department of Health Care Services (DHCS), Correctional Facilities, County Mental Health Plans (MHPs) and other County entities to ensure that the internal and external goals and strategies of the Justice Involved Initiative are in alignment.
The Justice Involved (JI) Liaison will be a subject matter expert of Enhanced Care Management specifically in the Justice Involved (JI) Population of Focus (PoF) including children and youth, pre-release services, understanding of Medi-Cal benefits and the concept of embedded/in-reach provider services and re-entry in the community.
The Justice Involved (JI) Liaison will be responsible for the cultivation and maintenance of effective relationships with relevant County partners to meet the needs of the Justice Involved (JI) population. Essentially, being the point person for the facilitation, implementation and troubleshooting of requirements and regulations as delineated by the Department of Health Care Services (DHCS).
CLASS CHARACTERISTICS:
The purpose of this position is to provide support to the Enhanced Care Management team for the implementation of, compliance to and the program operationalization of the Justice Involved Initiative and all its core components for participating ECM sites.

MAJOR DUTIES & REQUIREMENTS:

  1. Work with Enhanced Care Management (ECM) Leadership, Compliance and any other KHS entity to ensure that KHS is adhering to or working towards developing the systems in accordance with DHCS, NCQA, and other regulatory guidance.
  2. Be able to engage with relevant facilities during the warm transfer period and provide guidance to correctional facilities when a Member is transitioning out to the community.
  3. Work with Enhanced Care Management (ECM) Leadership to identify and bridge gaps and/or troubleshoot issues that the correctional facilities may have.
  4. Serve as the Subject Matter Expert (SME) for the Enhanced Care Management (ECM) Justice Involved (JI) Population of Focus (PoF).
  5. Facilitate building operational and effective working relationships between external JI ECM providers, ECM sites, and ECM leadership to achieve full integration of the initiative.
  6. Participate in activities that enhance correctional facilities and ECM leader relationships and collaboration.
  7. Attend Department of Healthcare Services (DHCS) , Local Health Plans of California (LHPC) and/or any other collaborative meetings held for the JI population.
  8. Provide awareness of Community Supports (CSS) benefits and any other relevant internal/external resource to ECM JI providers.
  9. Participate in meetings for potential ECM JI providers and in health services committees as relates to the JI PoF.
  10. Engage in ECM learning activity series to provide DHCS updates and training on the JI PoF.
  11. Ensure that KHS ECM and Counties are satisfying the requirements of the Memorandum of Understanding (MOU) for information sharing and efforts to coordinate care of KHS Members
  12. Represent KHS at meetings with the County Agencies and KHS community and provider partners.
  13. Participate in activities that enhance KHS and correctional facilities relationships and collaboration.
  14. Serve as a resource person and trainer to KHS Members, County staff, KHS team members, community agencies and health care providers.
  15. Work with Quality Performance and Improvement teams to ensure that our outcome metrics are captured and accurate.
  16. Perform other duties as assigned by Enhance Care Management (ECM) Leadership.

CORE COMPENTENCIES / KNOWLEDGE & SKILL REQUIREMENTS
  1. Knowledgeable in CalAIM Enhanced Care Management (ECM) and Justice Involved (JI) re-entry initiative.
  2. Agile and adaptable to changes from internal or external stakeholders for implementation of and operationalization of the Justice Involved Initiative.
  3. Must be able to work independently and maintain positive relationships with internal and external contacts at all levels.
  4. Requires ability to communicate and work effectively with a variety of providers and individuals with demonstrating problem-solving skills.
  5. Must be able to represent the KHS ECM Team in a positive and collaborative manner externally to relevant county departments, correctional facilities, community partners and to internal KHS departments.
  6. Must be extremely organized, with sharp attention to detail, having a strong work ethic and possess a willingness and ability to learn.
  7. Computer applications, including word processing, database and spreadsheets is required. Possess organizational skills to maintain accurate records and documentation. Proficient in Microsoft applications such as, Word and Excel. Strong oral and verbal presentation skills.

Employment Standards
Education:
Master's degree in social work, Public Health, Psychology, or closely related field from an accredited institution required.
Experience:
Minimum of three (3) years of experience working with the Justice Involved (JI) population, to include working with adults, children, and youth. Preferred experience performing or facilitating coordination of services preferably in an HMO, Managed Care or County Mental Health setting.
Certifications:
  1. Applicable Correctional Facility requirements per individual site requirements

  1. TB Test
  2. Influenza (Vaccination) or Declination
  3. Applicable Correctional Facility requirements per individual site requirements

Other
Possession of valid CA driver's license and proof of State required auto liability insurance. Required Travel Up to 50%
