Position Summary

Independent Living Systems is committed to enhancing member’s quality of life through innovative health and support solutions that reduce costs, foster independence, improve compliance, and enhance coordination of care. The Care Manager will work as part of a team dedicated to providing high-quality Care Management services to Members in our care, in a compassionate, effective, timely and professional manner.

Essential Functions

  1. Support members in the development of care goals by conducting assessments that uncover physical, mental, social, and Community Supports’ needs.
  2. Identify, initiate and complete referrals for social service programs, such as financial, community and state supportive services alleviating housing instability and other social determinants of health.
  3. Completes telephonic, video, and in-person interactions, including initial comprehensive assessments and ongoing contacts in a timely, engaging, and professional manner.
  4. Support, and review the performance of new ECM Case Managers, providing guidance, training, and mentorship to ensure optimal care management, member outcomes, and service delivery.
  5. Conduct ride-along visits with new Care Managers to members in their homes or where they seek care, and provide coaching on their interaction, assessment and care planning.
  6. Serve as a subject matter expert and provide guidance on service strategies for vulnerable populations.
  7. Assist in the implementation of program solutions to address Care Management challenges that require collaboration with external community-based organizations, managed care plan representatives, and senior leaders.
  8. Coach members and caregivers using evidence-based motivational interviewing techniques and trauma-informed care language to help members develop achievable self-management and model behaviors with new Care Management staff.
  9. Counsel and advocate on behalf of members; provide crisis intervention, as needed.
  10. Facilitate access to supportive services such as job training, life skills development, legal services financial resources, and healthcare providers, in collaboration with representatives of healthcare and community organizations.
  11. Maintain ongoing communication with members, caregivers, and family members to ensure engagement in the program, building and strengthening relationships.
  12. Effectively manage a caseload of 50 members, complying with workflows, documentation requirements, and timelines.
  13. Use motivational interviewing and trauma‐informed care practices.
  14. Monitor treatment adherence (including medication) and provide guidance and tools for the members to manage their own care.
  15. Respond appropriately (as specified in workflows and P&Ps) to DME needs, safety issues, emergencies, suspected abuse/neglect, and other critical situations.
  16. Document all services in eCare data management system in a timely and accurate manner, according to guidelines and workflows.
  17. Engages in effective transition of care from inpatient/SNF setting to home or different levels of care.
  18. Ensures proper continuity of care when cases are transferred.
  19. Develops and presents specific cases as part of case conferences and staff development activities.
  20. Participate in meetings, educational sessions, and conferences.
  21. Complete administrative tasks – read/respond to emails, maintain timesheets, and submit monthly expense reports, as well as research member issues through resolution under the direction of the Care Team Manager.
  22. Maintains vaccination records and active license or certification.
  23. Performs other duties as assigned.

Education and Experience

  • At least one year in the role of Care Manager with increasing levels of responsibility and case complexity.
  • Licensure/Certification: Current California LVN or RN license, Bachelor's, or master's degree preferred.
  • Minimum Experience: Demonstrated experience providing peer-level coaching &/or supervision. Two years of more of experience with vulnerable populations such as those who are homeless and chronic medical or mental Illnesses.

Required Skills

  • Competency utilizing the care management technology platform.
  • Proficiency in Microsoft Office Platform required.
  • Intermediate level of Excel skills required.
  • Advanced use of ILS’s eCareCentral system.
