ESSENTIAL FUNCTIONS:
To perform this job, an individual must perform each essential function satisfactorily with or without reasonable accommodation.
• Directly assist patients with scheduling appropriate follow-up visits before their discharge from the Emergency Department.
• Use low volume periods to contact patients who presented between CN coverage.
• Educate patients on the importance of seeking follow-up care and being engaged in their care journey.
• Record basic data regarding each patient interaction (e.g., whether the patient was able to schedule an appropriate follow-up visit).
• Help ensure patient follow-up by providing patient reminders (e.g., text, phone call, etc.).
• Support hospital strategy for the hospital.
• Maintain regular and reliable attendance.
• Perform other duties as assigned.
