Position assists in coordinating Community Health outreach programs and materials, with emphasis on education, group facilitation and promotion of programs and services of MyMichigan Health and the priorities outlined in the Community Health Needs Assessment and Community Health Improvement Plan., ensuring quality programming, meeting clinical best practice guidelines. Provides advocacy, support and education in area of overall health and lifestyle modification to patients and to community members. Facilitates support groups, meetings and educational events on topics such as Diabetes, Adult Obesity, childhood obesity, heart disease, depression, substance use, health screenings, etc. In coordination with the Community Health Supervisor, will plan and coordinate event attendance, event and educational planning and provide resources, support and training in health education. Assists in meeting grant requirements/reporting, interfaces with community partners/agencies and represents MyMichigan Health in community collaborative efforts.
Responsibilities:(50%) Facilitation of support groups, educational events and community agency meetings. Integrates knowledge, skills and experience to meet the educational and support needs of the individuals who participate in community education classes, support groups and/or agency meetings.
(25%) Assists in coordinating, planning and evaluating community outreach programs, community educational events, health fairs and event attendance, educational planning and providing resources, support and training in health education.
(15%) Develops, evaluates and distributes health information material related to, but not limited to, health focus areas described in the Community Health Needs Assessment and Community Health Improvement Plan.
(10%) Ensures improvement in best practices by assuming responsibility for self-development in life-long learning. Obtaining class facilitation certifications as need to implement strategies in Community Health Improvement Plan.
Certifications and Licensures:Preferred: RN - Registered Nurse
Required Education:- Education: Bachelors Degree
- Comments: Bachelor's degree in healthcare-related field or licensure and associates degree as a registered nurse. Master's degree is preferred.
EDUCATION, TRAINING AND SKILLS:
- Three (3) to five (5) years of experience in related field is preferred.
- Ability to teach/educate a group of individuals with varying education levels.
- Experience with project development and implementation.
